Reta user weighing a return to Tirz or a stack — seeking input

I switched over after being on 12.5 Tirz, and I've now spent 3-4 months on Reta, titrating as high as 8mg. The appetite suppression just isn't there for me anymore. I've put on 14lbs even though my workout routine hasn't changed at all — and I'm huuuungry, so I know I'm taking in more food.

Should I switch back to Tirz? Or should I lower my Reta to the starting dose and add Tirz back in? I'm feeling pretty discouraged and unsure what to do.

For context: I'm a female in my mid 40's. Tirz originally took 85lbs off me, but things had plateaued, which is what pushed me to make the change to Reta.
 
I'm still pretty new to Reta — 1 month in, at 4mg/week. Before this, I was on Tirz at 10mg/week. My schedule is Reta on Friday, then 7.5mg Tirz on Monday. I'm planning to bring the Tirz down to 5mg. The appetite suppression from it is something I really like. Things are going well so far. Nothing awful has happened, and it seems to offset the extra hunger that Reta brings on. Hopefully this is useful.

That being said, the suppression you get from Reta differs from what Tirz provides. Given where you are now — familiar with both compounds and how your system reacts to each — I can't think of a reason not to give a combination of the two a shot. Bringing 5-7mg of Tirz back into your routine might be what does it, though your own body is the only thing that can confirm that.

My own path was Tirz first, then I cross-titrated Reta upward, and I keep Tirz at a lower dose for its noise reduction. Best of luck!

It's been roughly 8 weeks for me. When I check my tracker, the reality is 5 months on Reta.

Appreciate it — I haven't come across much material about combining Reta with Tirz, so I'm trying to gather more details to build a protocol. I realize this pushes into grey territory, and stacking is something I've never done before. From what I gather, around here it's just standard practice.

That total silence of food noise while on Tirz — I had no idea. You really can't miss what you've never experienced.

I've also been considering a stack. That said, my reta dosing hasn't been very consistent. There is some appetite suppression, though it's weaker than I'd like, and I've gone as high as 12mg reta. Going that high leaves me overly fatigued. Over the past 5 mnths I've put on roughly 7lbs. Right now I'm just trying to figure out whether to go tirz only or do a stack.

So your approach was to hold tirz at a level you could tolerate while adding reta on top, increasing it gradually? At this point you've reached a clinical dose of reta plus a low dose of tirz? I'm thinking about trying a similar setup, mainly because of the side effects I've run into with each of them.

Since Reta doesn't do much for appetite suppression, it pairs well in that area. That approach helped me, though I ultimately returned to Tirz because allodynia showed up — maybe because I raised the Reta dose too fast.
 
What made you decide to change over? I understand reta hasn't been around that long and folks are eager to try it, but if tirz was working for you....why not simply go back to it? Over on reddit I've noticed quite a few people running both together, though another option would be to bring in cag to help with appetite control.

I began Cagri at a low dose (0.4mg) since the word was that it hit hard. These days I'm taking roughly 2mg each week, though I divide it into separate doses. My guess is that I don't respond very strongly to it. Aside from possibly feeling tired, I haven't noticed any negative effects from it. Even so, I'd take that over allodynia any day (being retired means I can grab a nap whenever I like!).

At 12.5 mg of tirz, I'd hit the top of my titration and the number on the scale froze. I decided to step away from calorie restriction for a while to see whether that would shake anything loose, but nothing changed. On top of that, I doubt I could handle a dose beyond 12.5.

Right around then I caught Covid, and shingles followed immediately after. Whether that played any role, I can't say, but once tirz stopped producing results, I began exploring other options.

Tirz has taken 85 lbs off me so far, and I've got roughly 40 lbs left to lose. I'm a woman in my mid-40s and fairly active — cycling, hiking, weightlifting, and I routinely carry a 50 lb feed bag without a second thought. Finishing off this remaining weight matters a lot to me.

My dilemma: if I simply go back to tirz by itself, won't I land in exactly the same spot? Should I remain on reta and add tirz on top? Or should I consider cagrilintide instead?

I've never run more than 1 peptide at a time, so whenever the conversation turns to stacking — which compounds, what timing — I get caught in a loop and can't decide.

Tonight I plan to dig into Cagri. Tirz was the only option on my radar since that's where I started, but seeing that Cagri has been effective for several people makes me want to look into it as well.

Wow, that's quite a stretch of time — at 8mg you definitely should have noticed something by now.

I'm still on board with bringing Tirz back into the mix. Sure, Cagri gets used a lot around here for quieting food noise (and it does seem to work), but here's my thinking: you already know how your body reacts to Tirz. Wouldn't that make it simpler to break out of the back-and-forth and just commit to running both Reta and Tirz?

Exactly! Naturally every body reacts differently, but so far this combo is treating me really well, and I'll only switch things up if I hit side effects I can't tolerate. The allodynia definitely waxes and wanes, but I'm fine pushing through it until I reach goal. After that, I'll figure out what my maintenance doses need to look like — could be tiny amounts of both, or maybe just some Tirz to keep making the smarter choices feel easier. We'll see.

For cagri, a more cautious initial dose would be 250 mcg (0.25 mg) or 300 mcg (0.3 mg).

Even the lowest dose in one study — 300 mcg given weekly — produced weight loss effects.

300 mcg led to a 3 percent reduction in weight, while 4.5 mg led to nearly 8 percent:

(0·3–4·5 mg, 6·0%–10·8% [6·4–11·5 kg]) versus placebo (3·0% [3·3 kg]

Sorry to hear reta hasn't kicked in for you yet. Assuming I weren't dealing with any side effects, my move would be to increase the dose. Since reta and tirzapatide don't convert 1:1 in terms of volume, there's no point in getting stuck on matching up the doses.

After bridging from Tirz over to Reta, I've done Tirz on 6 occasions for a cumulative 14mg. 4 of those 6 occasions fell on the same day as my Reta dose, and with 2 of those 4, I took them simultaneously with the Reta.

Beyond not having a clear dosing strategy behind it, I can't see any reason this would be problematic.
