I picked one up on Amazon, but the bottles end up resting sideways in it. Am I right that they need to stay upright? Because of that worry, I still haven't put the case to use

That's a curious story, though it doesn't really tie in. On our return trip from Joplin, Mo, TSA stopped my wife to inspect her carry-on. Inside they discovered a shaker plus individual baggies holding protein powder, collagen powder, and oat bran. After scanning, they took her aside (female officer) for a manual check and still couldn't figure out what the powder was using their expensive scanner. The chocolate scent was obvious. They confiscated it. This was a tiny airport with nothing but commuter jets. Leaving Ft Myers, Fl, a major international airport, gave us no trouble at all.

I fly with my cats, and they confiscated my little box along with a bag of kitty litter. While airports do have dog relief areas, there aren't any litter boxes, so I figured I'd pack a bit of litter in case our flight got delayed. In the end, everything turned out fine.
 
I take out my Amazon vial case, which has reconned vials and syringes inside, and I let them know what I'm bringing through and ask where they'd like me to place it...One told me to keep it in my bag, while others said to just put it in the luggage tub....Every time they do the 'step aside for a bag search', and they've only checked that the ice bag in it was fine...
 
I haven't done much flying with mine, though when I'm at the airport waiting for my bag to appear, I'll strike up questions with TSA agents—regardless of whether peps are packed or not. Their usual answer, more or less, is that it's a non-issue provided the supplies are within reasonable limits.

For my own kit, I rely on a compact amazon case that shipped with gel packs, and it's never caused problems when I've brought it along. I've also got a little zip-up pouch with "All My F**cking Meds" printed on it. Inside go the vitamin packs I assemble in small zip lock bags, plus my syringes. My instinct says that unless someone's hauling an absurd number of needles, the peps are basically beneath their notice.

Yep, gel packs—they really don't like those. After I fractured my leg, I was rolling in a wheelchair with crutches strapped on and a big gel pack. They nearly confiscated it, so I did the trembling lower lip thing and they ended up letting me keep it. LOL

And crying is a whole separate thing with women. 😁
 
On a return trip from Mexico, the small gell packs inside my amazon cooler got taken by their TSA. According to them, those packs would have been allowed if vials had also been in there. During my time in Mexico, I deliberately used up every one of my peps because US cistoms had me nervous. 250 mg. of NAD+ was gone 2 hours before the flight. Wow!!! That was a mistake!!

On a recent 2 week motorcycle trip, I brought along one of these. My V1 pen plus a spare cartridge, a 10ml Lipo-C vial, and one more small gel pack all fit inside. Each night I'd put it in the motel fridge to freeze, then pack it in the morning. It spent the whole day in a backpack inside my saddlebag with temps above 90°, and when I stopped it read 38°. One day I only refrigerated it overnight instead of freezing, and it still held in the 40's.

Whether it truly lasts the advertised 2½ days, I can't say, but for my purposes it did the job.
