4 months on tirzepatide: my progress report and what comes next

I've been on compounded tirzepatide for 4 months now, and the results have been mostly positive. I figured it was worth putting together a full write-up, and I'm also trying to figure out where to go from here — whether to stack something on top or just keep things as they are. Any feedback would be welcome. I'll keep updating this thread as things move along.

ABOUT ME: Male in my early 40s, active, but I've dealt with back and foot injuries, high cholesterol, and yeah, being fat. I'm 6 feet tall and have cycled between peak weights around 270 and adult lows of 210-220 more than once. Over the past year or so I've been hitting the weights much harder, put on muscle, and stayed injury-free with decent orthopedic health, but my appetite went up and keeping weight in check got harder. When I saw 265 again around January, I committed to starting tirz. My aim is steady fat loss while holding onto as much muscle as possible.

Below is a quick rundown of my numbers so far, then some thoughts and possible directions.

I usually weigh myself and inject first thing Saturday morning.

Week 1: 2.5 mg / 263.0 lbs

Week 2: 2.5 mg / 257.9 lbs

Week 3: 2.5 mg / 258.6 lbs

Week 4: 2.5 mg / 255.7 lbs

Week 5: 2.5 mg / 254.0 lbs

Week 6: 3.3 mg / 253.4 lbs

Week 7: 3.3 mg / 253.5 lbs

Week 8: 5.0 mg / 252.4 lbs

Week 9: 5.0 mg / 247.0 lbs

Week 10: 5.0 mg / 245.7 lbs

Week 11: 5.0 mg / 245.5 lbs

Week 12: 5.0 mg / 245.4 lbs

Week 14: 10 mg (split dose) / 243.5 lbs

Dosing Notes: My plan was to titrate slowly, both to keep costs down and to make it sustainable. I also didn't want to drop weight so fast that I ended up with loose skin or heavy muscle loss. This is just my experience, but smaller steps (like 2.5 to 3.3 mg) didn't seem to bring appetite suppression back, so I dropped that approach and instead moved between the standard doses at the slowest pace that still worked. During a short supply gap in week 14 I tried split dosing on the fly, but it felt about the same — no extra suppression compared to the once-weekly 7.5 mg — so I reverted. I think I'm ready to either go to 10 mg or possibly add 1 mg reta.

Side Effects: My first dose gave me a nasty headache and I felt off for about a day, but after that it's been mostly smooth. I still love food, and tirz doesn't kill my interest in eating or make me want to stop altogether — it just cuts cravings between meals and helps me stop sooner. I still catch myself grabbing snacks around the office, which I think is mostly habit — I snack far less when I'm not at work. I can overeat if I choose to, but indigestion kicks in quickly, so the tirz usually stops me. A few times though, especially later in the week before my next pin, I've still overdone it, particularly with dessert.

Other substance interactions: I haven't seen much talk about this. Tirz seems to raise my alcohol tolerance, probably from slower absorption, and it doesn't seem to make hangovers or recovery worse, so that's a plus. That said, I only drink now and then (1-3 times a month) and rarely heavily (no more than 5-6 drinks, often just 1 or 2). I often take low-dose edibles at night to relax or sleep, and I've read that might dull appetite suppression somewhat, but it's been fine for me so far. I'm beginning to look into peptides for sleep and mood too.

Sourcing: I'm not asking for sources here, but I hope it's fine to mention that I originally got my tirz from a US compounding pharmacy through a local doctor (very expensive), and to cut costs I recently moved to a US-based reseller, which has worked out fine so far. I might go more direct later, but I'm wary of the crypto side of things, and for now the US reseller markup isn't bad enough to outweigh the convenience.

Next Steps:

My target is around 220, maybe 210 if that seems doable once I'm closer. At that point I'll shift to maintenance and recomp. I'm considering a few paths:

1. Stay on plain tirz and bump to 10 mg

2. Add a low dose of reta rather than raising tirz, and see what happens

3. Go to 10 mg tirz and add tesamorelin for recomp / muscle preservation

The heart rate concerns with reta make me a bit cautious, but my resting heart rate is low and I haven't had serious side effects from other meds or peps, so I'll likely try it at some point — either stacked with tirz or as part of maintenance later. I thought about CJC/IPA instead of Tesa if I went that route, but most people seem to think Tesa works better and cost isn't a big deal for me. I'm also curious about other peps for support in a few areas, but I don't want to pile on too much at once. Thanks for any input — happy to answer questions.

Dropping 30lbs over 18 weeks is a real achievement. If progress keeps coming steadily, I would not rush to change the routine. At this pace, you are looking at 90# gone within a year.

What is great about a slower weight loss pace is that your muscle composition can still be preserved.
 
Nice job! When Tirz is doing its thing, keep using Tirz. Should it ever quit working, give it a couple weeks to see whether the plateau lifts. If it doesn't lift, then you begin considering alternatives such as Reta or Cagrilintide.
 
Great post, and the profile name deserves full credit too. My advice: steer clear of plateaus from the start by shotgunning Reta and Tirza.

I appreciate it! Sourcing isn't really the hard part—truth be told, I've already picked up a couple of vials and stuck them in the freezer as a backup—but for the time being I'm inclined to keep going with Tirz.
 
Your thread name, username, and that weekly chart are all excellent ... honestly textbook ... so congratulations! The Dad Bod idea is really fascinating. Some say men hit the gym not for their wife, girlfriend, or to attract a partner, but to earn respect from other guys. My wife completely buys into that and winces whenever I mention my targets for strength, size, weight, or body fat. She insists most women have no interest in a man with six pack abs, and that it would look strange on a guy in his 50s. I'm nowhere near a six pack, but whenever I spot an older guy at the gym under 12% body fat, I catch myself wondering where he parked the corvette — then I remember my wife is right, and that no women actually care about either.

haha, absolutely 100% with you. at the gym during my early morning slot, it's nothing but guys giving each other high-fives. as long as i can haul all the groceries in a single trip without keeling over, my wife couldn't care less.

This section of your write-up really resonates with me! A lot of people get pulled in by the notion that going low and slow is the “superior” approach, so they titrate at a crawl and then can’t figure out why the medication isn’t doing its job. The fact that you noticed the bump to 3.3mg wasn’t doing anything and went back to the study’s protocols shows real insight.

If it were me, before adding Reta into the mix, I’d keep pushing the tirzepatide dose upward. It’s clearly working in your case, you haven’t hit a stall, and there’s still plenty of headroom to go higher. When something is already working well, what’s the point of introducing an unknown?
 
That makes sense to me. When tirz is doing its job, side effects are minimal, and the goal is to drop roughly 50lbs — around 20% of where you started — then there is no reason to change course; just stay on tirz and bump the dose when necessary. It really does seem like tirz can carry you all the way to your goal before you hit the ceiling on dosing, and keeping things uncomplicated usually beats adding layers.

Male, 40 years old. My starting weight was 262.2 back in mid October. These days I'm running 2 mg of Reta plus a 1 mg sema dose mid week. Sema was what I began with, then I moved over to R. I've still got S left over, so I'm just using it up. As of this morning I'm sitting at 209.5.

Keep at it! Slow and steady wins.

Week 19: 0.0 mg — I forgot to bring my shot with me when I headed out for Memorial Day weekend, so nothing went in. Total doofus move.

Normally this is the day I pin, and right now I've got a headache that I suspect is from my blood sugar spiking. Anyone else here ever dealt with side effects after skipping a dose? I'm not getting back until Monday afternoon, so the plan is basically to pin the moment I walk in the door.
 
Week 19 (+2 days) - 10 mg, 236.0

Week 20 - 10 mg, 233.7

That 2 day gap hit me harder than I expected — felt off for a few days on either side of the late pin. MD weekend also meant barely any gym time. Trying to cut myself some slack. Things have mostly evened out over the past week or so. Next week I'll likely pin on day 6 so I can slide back to a Sunday schedule.

There's a woman in the office near me who keeps a huge candy bowl on her desk, and even though I'm not hungry I keep grabbing some whenever I walk past. If she got fired I'd probably be at 190 already 😂 Portion sizes at meals have definitely gotten easier to manage since the dose went up, but if food is sitting out during the day I'll still graze.

Starting tomorrow I'm going to run a vial of KLOW for recovery and gut health. First few days will be 1.5mg so I can check for any major ISR problems. I've used wolverine before, and from what I hear KLOW causes fewer issues than GHK-Cu alone, but I'm easing in regardless. The plan is to pin in the morning, after the gym and before work.
 
Hi Al, I’m fairly new to the wolverines stack. I have a question: my 20mg 10/10 mix has a yellow tinge. Is that normal? I’d rather check before I put something into myself.

Week 21 — 230.4 lbs at 10 mg (+3.2mg KLOW daily)

Week 21 — 228.4 lbs at 10 mg (+3.2mg KLOW daily, 500mcg Semax NA intranasal daily)


The 10mg dose looks to be doing the job. I've got a 2 week work trip in mid-July, and I'm really hoping to be at my initial GW of 220 by then, then reset / reassess. I added Semax NA this week to see whether it eased any of the anhedonia / brain fog that comes with a cut this long — I do think it's helped a bit, though it's pretty subtle for me.

Sleep has been a touch worse this week — could be the Semax, could be the rising tirz, could be something else entirely... My plan is to run the Semax solo for a week to gauge it, then bring Selank NA in on afternoons during the 2nd week, since plenty of people run the two together. I've also ordered Adamax and Bromantane, and I may cycle each once the Semax runs out to see which I prefer.

On the whole, I expect to reach GW soon and then move more deliberately into recomp / maintenance, probably bringing in some GH secretagogues and/or reta at that stage — but for now I'm holding the course at the current tirz dose.
 
Reta + Tirz + tesa + kpv is the stack my RS likes best. Appetite control comes from Tirz, reta bumps up metabolic rate, sleep and recovery get a hand from tesa, and inflammation is held in check by kpv — overall a fantastic stack for body recomp and feeling great

The KPV I'm using comes bundled in the KLOW, so teasing apart what it's actually doing versus the rest of the blend is basically impossible. I've never tried KPV as a standalone.

When I kicked off KLOW I snapped a "before" picture, hoping that after a couple of months I'd be able to spot some kind of change from it. Realistically though, that might only amount to "keeping a record of whether your skin improves once you've got a tan," given that it's summer now 😂
 
Cut out alcohol and the remaining pounds will come off quickly! It was the best choice I ever made. I also kept my dose increases as gradual as I could. I started in November and only recently moved up to 6.25mg. My weight began at about 225, and now I'm at 193 and beginning to look lean
