Switching from tirz to reta - what dose to start on?

Right now I take Tirzepatide at 15mg each week and the results have flatlined for me. My plan is to move over to Reta, so I'm after some guidance on a sensible starting dosage. Clearly I don't want to go too hard too soon, yet I also have no interest in spending months creeping up before I reach a dose that actually works. Any advice would be much appreciated!

What about stacking a small amount of reta with your Tirz?

That hadn't crossed my mind, tbh. My tirz supply has maybe a couple of weeks left on it, so the idea was simply to switch over rather than order more tirz. Have you run the two together?

It's come up on this board a great many times already. Spend a while reading through before posting. I suspect everything you need is already sitting in those threads.

The usual approach to the changeover, as I understand it: bring the Tirz down gradually while the Reta goes up over the same period.

A lot of folks "stack" reta alongside their tirz — either cutting the tirz and putting reta on a different day of the week, or just topping up their existing dosage with small amounts of reta on other days.

Either way, you'll probably still be on tirz for more than a few weeks while your body adjusts

plenty of threads cover this with all sorts of answers.

Splitting the dose is likely the gentler route into it…add 1-2 mg of reta each week while bringing Tirz down over the same stretch…or simply add Reta if the goal is breaking through a plateau

I'd back that completely — run a search through the threads that already exist for "plateau", "stack" and so on. Starting a fresh thread for what is the same question over and over is disruptive, and it doesn't help the other members who are in your position.

With that in mind, work out whether your hunger and food noise creep back in around the five or six day mark -- if they do, split-dose your tirz (try searching "split").

Once that approach stopped helping me, I kept the tirz at split-dosed 15mg/week and started adding reta on a slow upward titration. It did work, though at 3mg the anhedonia (search) it brings is only just bearable for me. As soon as I get to goal weight or close, my first move will be to take the reta back down.

Better to live with side effects you already recognise (and can reasonably put up with) than to gamble on new ones. Any med can produce negative effects, big or small, right away or later, obvious or sneaky.

When I moved off Tirz and onto Reta, I called that stretch my "bridge".
As I remember it, didn't you "bridge" in an EOD sort of way?

Your way of building levels up seems to involve more than one pin a week?

Daily is the only way. EOD is a grandma schedule - pin it every day and become the reta bro you were meant to be.

Fair enough. Thanks for taking the time to reply, and sorry for adding a post that's redundant. I’m still new around here and “searching” isn’t really my strength. A few attempts turned up nothing, so I clearly need better keywords — what I was after never showed up.

Adding reta on top of tirz, taken from @lessthanhalf - EOD:

I did it backasswards losing the weight then starting GLP's, but when I got to 15mg of tirz with very minimal side effects after a year of nausea on ozempic, and I was still hungry, I did not want to stop tirz and switch to reta as it was working pretty well , so I just added reta 5mg, and much later added in cagri at 0.25 mg every 4 days. I was dosing smaller amounts every 2 days to minimise side effects, mainly because I had to do that with ozempic due to the nausea, so just stayed in that pattern out of habit mostly.

But that combination is working for me , I lost another 13 kilos or so to get to 65 kg after originally starting at 145kg, and losing most of it before starting them. But it is a huge weight loss to maintain , which is why the high doses and combos seem to be needed.

I was using 2nd daily doses which does make it much quicker and easier to adjust doses up and down , but it could just as easily be done with weekly doses but would be better done fairly slowly. I definitely noticed mild nausea from 0.25mg of cagri and spent a month or two trying to work out if I liked the slightly reduced hunger with slightly increased nausea or not.

Adding in reta again might be worth trying at a much lower dose, but logically low dose cagri probably makes more sense

Transitioning off 5mg of Tirz (Tirz began on 12/12/25)

Trials are pushing Tirz past the 15 mg mark as well - 20 and 25mg as far as I know. One person reportedly ran 30 mg and lived to tell it.

There's a guy on here who's been around forever and he runs roughly 80mg of tirz weekly..
