Sudden Tirz intolerance — did anyone else hit this?

After 5 weeks at 3 mg, it all landed on me in one go, as though I'd injected 30 mg instead of 3 mg. Same vial, NO dosing mistake, no math errors on my part.

My stomach more or less factory-reset itself. Nausea hit me like nothing I've ever been through, along with extreme weakness and a general sense that everything was doomed.

Over the last 10 days I've hardly managed to stand up straight. Even small movements leave me short of breath and tachycardic, as if every trip to the kitchen were Olympic training. Blood sugar on the low side (not dangerously so), blood pressure low as well.

And to make it stranger still, I came down with a monster cold at the same time and ended up on antivirals for suspected shingles/herpes zoster, thanks to this awful “needles in the skin” sensation I had.

Except… there was no rash. No blisters. Nothing. So it seems even my maybe-shingles wanted to stay mysterious and dramatic.

The doctor essentially shrugged and told me to skip last Thursday's dose. Bloodwork comes back May 20th.

Today Shotsy puts my level at 1.48 mg and I'm marginally better, which means I've graduated from “barely functioning houseplant” to “weak but technically alive.” I still hardly eat or drink, mind you.

What worries me now is that I somehow turned suddenly and permanently intolerant to Tirz overnight. I can't really find stories like this anywhere, so I'm asking: has anyone had a sudden hypersensitivity or reaction like this after tolerating the dose perfectly well before? Any theories, experiences or reassuring stories are welcome.

For context: Tirz has always been a real struggle for me, and I came close to quitting outright. Then you guys pointed me at microdosing, and that made everything much easier to handle.

Even so, it has never been an “easy” medication in my case — but the results justified putting up with a bit of suffering.
 
That prickling, sore skin reaction can occur with either Tirz or Reta.....though it appears to show up more often with Reta. It sounds a lot like a Reta overdose. Are you certain it is the same vial and the same dosing etc? Nothing else new has been introduced?
 
Yes — I went through this myself last summer while using standard prescription Mounjaro. Right after the injection, out of nowhere, I got hit with nausea worse than anything I'd had before, plus severe diarrhea. Staying hydrated was a real fight because of the nausea, and eating was completely off the table for an entire week. My dose at the time was higher (7,5mg), but I'd already been sitting at 7,5mg for 5 weeks, and it was the very same pen I'd used during the 2 weeks before that.

The reassuring part (based only on my own experience) is that it doesn't stick around forever. I quit abruptly because I was terrified of a repeat, and stayed off for roughly 4 months. Eventually curiosity about grey got the better of me, so I restarted at a low dose (Tirz 2,5mg) purely for maintenance and to quiet food noise. Once I'd been on that low dose for a while, I chose to go up again, since I'd never reached my goal weight and was still obese. Since then, by titrating up gradually, I've dropped another 5kg.
 
You mentioned there was no dosing error, but honestly this really comes across like one. Stories about dosing errors are everywhere, while accounts of intolerance appearing suddenly are basically nonexistent. I've been completely certain about things before and still turned out to be mistaken. A lot of people have described forgetting a dose was already taken and doing it again, or simply measuring out the wrong amount. If I were in your position and things had been going well, I'd likely hold off until I felt better, then restart at a slightly lower dose and observe what happens.
 
I realize you said it isn't a fresh vial, but could it have accumulated in your body by now? Is this a new kit from another supplier you've never tried? I've come across reports of people experiencing sides with certain ones but not others.

My dose went up to 5mg 2 weeks back, and within a few days nausea set in and I've had zero motivation to do anything all week. Malaise, I think that's the term. Eating has become something I have to make myself do. Across all 17 weeks of using it, I've never experienced anything like this. At first I assumed it was dehydration, since that's normally how dehydration hits me, so I pushed my fluid intake way up. I went heavy on hydration drinks and took in so much salt that my hands are now puffy. Since last week I've dropped every supplement except magnesium and potassium, and this weekend I even cut out the GHK-cu, yet I still feel awful.

A month ago I began a new kit, and during the first 2 weeks it seemed to do almost nothing, which is exactly why I raised my dose in the first place.

Today I'm feeling a little bit better, but my plan once this passes is to divide the dose and do 2.5mg every 3.5 days to find out whether that makes a difference.

Yes, I’m certain: the vial is the same, the concentration is the same, my dosing routine is the same, and I’m confident no dosing error occurred.

No Reta and no other new peptide has been introduced. There’s nothing new in the mix at all, apart from the antivirals, and those only began once the skin symptoms had already shown up, since my doctor suspected shingles/herpes zoster.

I did already know that Reta can bring on the “needle skin” sensation, but I hadn’t heard of it with Tirz. Now I’m beginning to question whether that was Tirz as well. What’s odd is that it showed up only on my left side, which is exactly why shingles/herpes zoster was suspected from the start, given that it wasn’t bilateral.

A fairly nasty cold and a stretch of heavy stress were definitely part of the picture, yet the reaction still came across as wildly out of proportion.

So that’s why I’m left so puzzled: it genuinely resembled an overdose, but I can’t come up with any real-world reason why one would have happened.

From what you describe, our situations line up closely, though my symptoms leaned more toward nausea and feeling weak rather than diarrhea. That bit about struggling to keep fluids down? That has been my exact reality.

Hearing that it eventually resolved for you means a lot. My head keeps spiraling right now, telling me my body has written off Tirz permanently as something toxic…

I'm on day 12 as of today and things have only eased slightly, so injecting again is completely off the table for me at this point 😂

Did you ever think about switching over to Reta? Honestly, the thought of returning to how loud the food noise used to be long term is something I can't picture.

It's great that you were able to start up again without issues and keep losing by going back up on the dose gradually.

I get why that thought would cross your mind — if I were on the outside looking in, a dosing slip would probably be my first guess too 😅

Which is part of what makes this so baffling for me: what I felt was close to what an overdose would look like. I also can't stop asking myself if some sort of buildup could be at play; at this stage, I'm out of ideas.

I went over the dose with a lot of care. Tirz has always been something I handle very cautiously, since my body tends to react strongly. Past side effects have made me almost obsessive about getting the dose right; if there's even one click of doubt, I'd sooner toss the whole thing than gamble on a week that's far worse than my usual.

What you're suggesting sounds reasonable. I truly do want to keep going — it has changed my life, and I finally felt like my food noise was under control. So whenever I restart, I'll take your advice and go with a lower dose, moving ahead very gradually. My only worry is that I've somehow developed an intolerance to it.

The reaction and symptoms from Tirz were clearly the first thing to appear. A few days afterward, the antivirals were begun due to what was suspected to be shingles/herpes zoster.

And you're right that antivirals can be harsh too, which, honestly, only makes this whole situation harder to sort out 😭

To be honest, right now the idea that it built up or accumulated over time seems like the most plausible explanation to me, particularly when you factor in stress and illness happening at the same time.

And to be clear, I hadn't switched to a different vendor or a different batch. It was the exact same source I'd been using and handling fine before this all started.

What confused me the most was how abrupt and out of proportion this reaction was next to the Tirz side effects I normally get.

Yes I have, and honestly the only thing stopping me is that I'd already bought several kits of Tirz. Once those are gone, or if I can't reach my goal weight, I'll likely revisit the idea.

That "allodynia" feeling—like needles or sunburn—doesn't have to show up on both sides to be caused by Tirz (or Reta). I've had it with both peptides, and it pops up wherever it feels like it: ankles, tops of the feet, upper thighs, glutes, back, stomach, and even my cheek/face (right side only). Before blaming shingles, I'd pin it (pun intended) on the Tirz.

Honestly, what you're describing lines up almost exactly with my own situation — particularly how it crept in late and left me thinking "what on earth is my body doing right now."

The whole malaise and apathy thing hits home hard for me as well. Nausea alone doesn't cover it; there's this bizarre whole-body sense that I'm barely able to get through basic human tasks.

At the start I figured dehydration and electrolytes were probably the culprit, so I went hard on fluids, yet that still couldn't account for how bad things got.

Your point about going up because earlier weeks seemed "too mild" is fascinating as well. It genuinely has me questioning if these peptides can produce some sort of lagging buildup or saturation effect.

If I do start again, maybe I'd try splitting my microdose right from the beginning.

And also: I kind of adore you for describing such a close match to what I've been through, and I truly respect that you got through this without missing a single dose, since at this point I'm so rattled I can't even bring myself to glance at a syringe.

… because I didn't know any better, the only “needles/allodynia/sunburn skin” reports I'd come across were tied to Reta, never Tirz. So once it showed up on just 1 side, I fully bought into the shingles/herpes zoster theory.

“I would pin it on the Tirz” — that line deserves an award lol

On a serious note, thank you so much 🫶 Tomorrow morning I'm definitely bringing this up with my doctor… though I doubt I'll be thanking him for the 7 days of antivirals that might have knocked me down even harder.

What keeps making me think Tirz is the culprit: alongside the crushing fatigue, the symptoms are extremely particular — intense nausea, acid reflux, profound gastric stalling and fullness, and being unable to eat or drink in a normal way.

These line up almost exactly with what hit me when I began Tirz in the first place — only now they've been dialed up to an extreme degree

Yeah, I think it's possible the antivirals piled on and amplified what was already going on lol

Thanks a ton 🫶 I'm really hoping this nightmare begins to lift before long as well
