Stalled at week 8 — hold at 3mg or jump to 4mg?

8 weeks into Reta now, and this is the first one where nothing has moved!

The opening 4 weeks were @1mg

The next 4 have been @2mg

Every week until now I dropped 1kg.

My intention was 3mg for the 4 weeks ahead, but with the scales stuck this week im

So now I'm debating whether to simply jump to 4mg?

The plan had me doing one week at 3mg and beginning an 8 week Tesamorelin course in my 2nd week @3mg

Do I keep to it, or jump to 4mg — something I wasn't going to do for another 4 weeks?
 
This is only based on what I have gone through myself. When I moved up from 2.5mg straight to 4mg, the weight loss result improved noticeably. My sequence has been 1, 2, 2.5, 4, 5, and at present 6.
 
For my RS, I'd stay at 3 for 1 week first, check how it went (whether it made a difference and whether it was tolerated well), and then, if I wasn't happy with it, go up to 4 the week after.

With reta, moving up in dose sooner is absolutely an option — so far you've played it pretty safe.

That said, I'm wondering about your tesa approach. What's the reasoning behind 8 weeks at 3 mg? Not a knock on you, it's just that I've never come across that particular regimen as a therapeutic recommendation. What I've consistently seen and personally followed is 2 mg, dosed 5 days on and 2 days off (the off days let receptors reset, which matters) across a 6-month span. That's the protocol used when Egrifta (tesa's brand name) is prescribed. Going with 3 mg isn't going to cause harm, but my memory is that clinical trials showed no extra visceral fat reduction once you go past the 2 mg dose. (That just cut your tesa cost by 33%. You're welcome.)
 
In my case, going from 2mg up to 4mg produced the same result as 3mg — roughly 3lbs per week. Today I'm injecting my 4th dose at 4mg, and on Monday I'll make the call on whether to remain at 4 or bump up to 6-8mg, based on what the scale shows.

As for the stomach problems, they've pretty much disappeared now that I've done 8 weeks on Reta.

I began a low carb way of eating on 1/9/26. I don't count calories or protein or anything like that. My morning meal is either a hard boiled or fried egg plus a sausage patty that my wife prepares, midday I have a protein shake, and in the evening I eat some kind of meat (fish, beef, pork, chicken), cottage cheese, sliced tomato or steamed veggie. I typically drink 2-4 beers daily.

My weight at the start was 215lbs, and at Monday's weigh in I was 195lbs. The goal is 175.

How you titrate comes down to the side effects you're experiencing where you are now. When any are present, move up gradually to 3mg; when there are none, go with 4mg. Doing it that way is logical, though it still doesn't promise that a higher dose won't bring new side effects.
 
I share @woundcarping's view that the outcome probably won't differ much either way, but..... I'd argue that hitting an "off" week is fairly typical even when using GLP1's alongside any kind of calorie deficit. That said, if it stretches to 2 or more weeks, or if you track weight frequently and keep meticulous records, then it might be worth examining more closely.

Beyond that, it genuinely irritates me that none of these threads ever bring up diet or calorie intake. It's a bit like missing the forest for the trees. I put the blame on how inexpensive these drugs are.

When you’re already dropping 1 kg weekly, the dose is performing as intended. A single week of no change doesn’t mean a plateau—it’s ordinary body variation. Moving up in dose too quickly tends to bring more side effects and can reduce how long the medication works. In this case, slow and steady genuinely comes out ahead.

What about people also often omitting their starting weight, current weight, target weight, and goals?😂

It’s likely wiser to increase the dose more gradually. That doesn’t mean going straight to 4 is off the table, but the side effects might hit you harder. Also, have you changed your eating habits at some point? That can sometimes make a big difference while you’re moving up in dose.

You've got it 100% right! At only 8 weeks in, with this being the first week without a drop, it's easy to let emotions take over. Don't let it rattle you—you have to think long term instead of making knee jerk reactions, and always err on the side of caution. Slow and steady will give you better long term performance.

I fired that post off in a hurry while juggling work 🤫😂 but I should have included more details either way.

Tesamorelin is dosed at 2mg 5/2. That part was just written poorly.

Starting weight 105kg

Current 98kg

Target 90kg

800 calorie deficit

Weights x 3 per week

Cardio x 2 per week

I'm away for the weekend right now, so the diet will be out the window, which maybe is the reset the body needs?

I think I'll jump on 3mg as planned, slowly slowly catch a monkey ☺️
