Raise the Dose or Add a Stacked Compound?

I've stayed at 15mg of tirz for roughly a year at this point. The results aren't what they used to be — I can't drop weight anymore, and keeping what I lost is a battle. Early on I dropped 85 lbs, so it clearly worked, but now I'm stuck wondering what comes next.

Adding Reta makes me nervous because I have chronic kidney disease, and I've come across info saying reta should be avoided in that situation.

My food noise is awful. I really miss the days when food wasn't constantly on my mind. Should I simply raise the tirz dose? How high can I go? Besides reta, what else is out there? Would stacking sema be a worthwhile option?

Thanks!

There are people in this community who have pushed past the 15mg ceiling that EL established. Research on larger doses is also underway.

Definitive answers are genuinely tough to come by here. Responses vary from person to person.

A different approach involves lowering your dose slightly and injecting every 4 or 5 days. That's what I do, since suppression used to wear off by day four and days 5 and 6 were rough for me. Prior to trying that, bumping up to 15mg brought on sides. Low-dose Survo also gave me sides when I introduced it.

If your main issues are appetite and food noise, Reta probably won't do much - I'd look at stacking Sema or Cagri instead.

How are your sides? A trialed max dose could be higher than what an individual can tolerate… and i suspect the reverse also holds.

12mg weekly of Reta is where I am, with no adverse sides. A grey market non approved peptide is what I take… nothing I’ve seen or experienced suggests a reason to not escalate dose as tolerated. I’m not beholden to the trials. Going beyond 12mg/week holds no apprehension for me.

Where tirz has been working comfortably… increasing dose seems easy, unless you already have other options at hand.

Has sema been something you've used? It does a great job of quieting food noise. Personally, I'm enjoying combining sema and tirz.

Every now and then I'll put a small amount of Sema into my Tirz shot, which does cut down the food noise and hunger.

Even so, based on the clinical trials, weight loss tends to plateau or level off at roughly the 72-week mark, so it's possible you've simply hit the ceiling of what you can lose.

That makes sense. The biggest issues I deal with are tiredness and occasional headaches. Those haven't shown up recently, though. Typically they came up right after upping the dose.

I've been thinking about giving that a shot too. What's your approach?

That does sound like it could work. I'm guessing your Tirz stockpile is pretty large? What about Cagri — do you have any? Compared with sema, which leans more toward GLP, Cagri would act on a different receptor.

Starting from week 3 on GLP, I've been dividing my dose into 2 shots each week (that's been going on for the last 11 weeks). Right now I'm averaging 2.8lb lost per week while sitting at 242lb. I've got a good amount of cagri, sema, tirz, and reta on hand. For my next move, the plan is to move up to 14mg/week.

Once I'm comfortably at my goal weight/condition, one idea for a plan is to taper down. Another option I find more intriguing is taking roughly a 2-3 month break/reset and then starting again at a lower dose. I'm paying attention to what other people say about their resets, and since I've got lots of time, I can keep working out what my plan will be 😂

My tirz supply is fairly solid, though it's the only thing I've got. Before buying anything new, I wanted to figure out what might work as an addition or a replacement. A bunch of folks recommend stacking sema, but I'm uncertain whether that would deliver the same results as cagri, or whether it'd be better to just move over to sema entirely.

I've only just gotten my tirz and I'm still testing it out. Even so, at 1mg sema together with 2.5 tirz, eating is almost impossible for me.

For a while now I've been on roughly 17mg of tirz, and I haven't hit any bad side effects. Around late November I actually began adding sema on top. The sema dose was low, around .5mg or 1mg, and things went great until my sleep fell apart. I figured sema was the culprit, so I dropped it for the past couple of weeks. My sleep hasn't recovered, which makes me think sema wasn't to blame after all 🤣.

Given that you've spent a long time at 15mg of tirz, my guess is adding sema would work out better than pushing more tirzepatide. Between 15mg and 17mg I honestly couldn't tell much of a difference. I had intended to move up to 20mg, but after trying the sema stack I believe that route made more sense.
