PT-141 round 3: when it would not quit

Yesterday PT-141 hit me harder than expected, so I figured I would post about it in case someone else is trying to figure out their dosing.

Two earlier tries were at lower amounts: 0.5mg came and went without any effect, and 1mg did the same. That is why I went with 2.5mg yesterday. Big mistake. It started working around the 45 minute mark. Once I had... handled things... it stayed up anyway. I am talking about 6 hours, possibly longer because I nodded off while still hard. Food was also the last thing on my mind, and when I got up this morning I felt wrecked, like I had done two marathons: sore all over, everything.

Takeaway: only move up by half gram steps 😛
 
Honestly, take a look at the post I linked, plus the article that goes with it — I've put both below so you don't have to hunt them down. What the headlines leave out is the part that actually matters: "the last erection you may ever have". Fingers crossed you dodged it. Different compound, sure, but that doesn't guarantee anything — the mechanism could still line up. Watch yourself going forward!

Is everyone really using PT-141 without first looking into priapism and remembering that once 4 hours have passed, you should already be heading to the ER?

Yes, I had done my homework, and I knew the guidance about heading to the ER at the 4 hour mark. I've experimented with other "male enhancement" products before, so a stubborn erection wasn't a surprise. Everything is functioning normally today.

Hey, that's your call and I'm not going to tell you otherwise. My only concern is that someone who's more careful about his Johnson might come across "popped a stiffy for 6 hours, passed out, haha what a ride" and not realize that's, you know, something you should get to a doctor for.

True ischemic priapism isn't the typical outcome of PT-141-induced nighttime erections. The real risk lies in stacking PT-141 with other drugs, such as antipsychotics or trazodone:

A man in his mid 40's, sexual function totally fine, but curiosity about how PT-141 would "feel" led him to inject 1.75MG IC the previous night, roughly 3 hours prior to bed and about 1 hour after eating dinner. Nothing at all was felt. After listening to music for 3 hours, he drifted off...

Yep, Viagra 2 decades back gave me an erection that was painful. The one this time was just annoying more than anything.

These strategies lack sound evidence, but the risks of attempting them are minimal as long as access to more definitive treatment is not delayed. Lower-limb exercise as a first-line treatment warrants further study in the undifferentiated emergency department priapism population. The case we present and discuss here illustrates the potential benefits of a trial of acute lower-limb exercise, specifically stair climbing, as a treatment for medication-induced priapism.

What Huberman brought up was melanotan. Bremelanotide—sold as Vyleesi, also called PT-141—holds FDA approval.

I'm trying to reconcile these 2 things. Is a whole-body ache something you get simply from using it? I'd figured the soreness came from pushing your body to the point where all your muscles hurt, but maybe I've got that wrong. Or maybe the way you act as your own partner amounts to a full-body workout?
 
What started as a general post so others could learn from my bad experience and make smarter choices when testing PT-141 turned into a TMI info dump 😛

That intense muscle soreness the next morning caught me completely off guard, so I looked into it, and it turns out this is a known side effect that does not show up very often. One source even called it "flu like muscle aches," which is precisely how it felt to me.

Worth mentioning: the soreness and that nonstop urge to stretch the muscles both cleared up within a few hours.
