Moving from a 5mg Mounjaro pen to grey tirz — how should dosing be handled?

Hey everyone, I’m on a 5mg Mounjaro pen right now, and I want to get a clearer picture of what to expect if I switch over to grey tirz

There are a few points I’m hoping people with firsthand experience can shed light on:

When the product is properly dosed and verified, do the effects tend to line up?

Should someone usually stay at the same dose (for example, 5mg), or is it wiser to begin at a lower amount after making the change?

I’m not asking for sources — I just want to understand the best practices and go about this as safely as I can.

I can't really speak to a direct equivalent, since my Mounjaro came from doctors and I later switched to Reta after doing my own research. For me it's been better all around and less expensive through grey markets, and at 2.5mg of Reta I feel the same as I did at 10mg of Monjaro, buddy.

So my suggestion would be that staying at the same strength should be okay, because it ought to be like for like as long as all the proper testing was done (same purity and so on). I figure they're all sourced from the same place regardless — we're just paying the absurd markup and the fancy labels that come with a pen lol..

#1 - Products from the grey market tend to contain more than the label states, and they aren't always the same. To give an example, the very 1st grey order I placed was supposed to be a kit (10 vials) of 10mg semaglutide (ozempic/wegovy), but testing on the 3 vials that were submitted showed an average of 12mg.

#2 - When someone is on a GLP1, that extra fill generally isn't a big deal. To go back to my first order as an example .. had my goal dose been 2mg and I dosed based on what I purchased (10mg), every vial would have provided 5 doses, yet because the vials actually averaged about 12mg, the dose I'd be injecting would be nearer to 2.4mg rather than 2mg. I'd guess that additional .4mg could be significant for someone who deals with an annoying side effect, but if they know going in that it's overfilled, they can plan around it.

For you, suppose you get a kit of 30mg tirzepitide from the supplier I normally use. The most recent batch they had came back at an average of 35mg. Your goal dose is 5mg, so every vial would yield 7 doses. (whether you keep that vial for 7 weeks is up to you; officially nothing should be kept beyond 28 days .. but hey .. your call) If you base your doses on the amount advertised, your vial would instead give 6 doses of 5.8mg rather than 5mg. All things considered, I doubt that extra .8mg would make much difference.

Anyway ... that was my long-winded way of saying ... keep an eye on the overfill if you think it matters.

I kept my dose unchanged when I made the switch. Regarding the overfill, MsGizmo is right. Vials tend to contain slightly more than the label states. That hasn't caused me any issues yet.

Should it concern you, an option is to divide the dose across a couple of weeks. Splitting is something I already do on a regular basis. For instance, 2.5 on Sunday plus 2.5 on Wednesday gives you 5.0 for the week. That way, any extra overfill won't hit you in one go. Just a thought.
 
The amount a vial holds above or below its label is nearly always within 10%, and usually that difference has no real impact. The one case where it could matter is when even a slight bump in dose sets off side effects for you.
 
I began with Zep and worked my way up to 10mg. Then I moved to compounded, kept the same dose, then went to grey, again the same. Across every source I stayed at 10mg. Everything was largely identical, with the one exception that the additive in the compounded version didn't sit well with me.

My wife and I went straight from MJ at 5mg to grey at 5mg. No noticeable change at all — aside from the insane gap in cost, naturally 😉

If anything differed, it leaned toward the better side, so to speak, because grey doses often run a touch stronger. That happens since vials are frequently filled a little above the label — take a standard grey "TR30 vial" (30mg/vial), which commonly holds somewhere between 31 and even 36mg of Tirz.
 
I went through the same change. What I did was drop the Mounjaro pen down to 2.5 until the vial was empty, then combine that 2.5 with the grey and bump it up to 5 once I was finished.

It's pretty much the same thing.

Another option, if that's simpler for you: get a pen plus a 3ml cartridge and use it just like the Mounjaro pen.

Try not to obsess over hitting the dose exactly — you'll land more or less on it, and being off by +-0,5-1mg isn't a big deal
