Is there anyone taking tirz at a maintenance dose? How does that stack up against the dose you used while actively dropping weight?
 
man, this tab's been sitting open for 24 hours and I was hoping to pick something up! Is nobody doing maintenance?

In the Phase III Surmount-Maintain trial, subjects who continued at their highest tolerated dose held their goal weight without regaining.

When participants stepped down to a 5mg maintenance dose, they regained 30% of what they had lost — in other words, they kept 70% of the total reduction.

An independent study from Scripps kept people at goal weight on the same dose but stretched the time between shots to 14d. Worth noting: those patients were using either Tirz or Sema. By the end of that study (4 mo's later I think But not sure?), they had dropped another 2-4lbs on average. Fair warning, it was a small study — just 30 people.

Another route: someone I'm assisting chose to step down to 2.5mg/wk for maint (his max was only 3.5mg/wk since he didn't have a very large amount to lose, 22lbs). For the last 3 weeks of maintenance, that's been working for him.

My maintenance regimen is 2.5 mg of Tirz taken every 5 days. When the holidays come around, or when my eating habits slip, the scale tends to creep up by a few pounds, so I bump it to 3mg every 5 days. That said, it's possible I'm a super responder, since the highest I ever managed to tolerate was 7.5mg every 5 days.
 
Right now I take 6mg of Tirz every 5 days to maintain. My weight has stayed within +/- 2lb for the past 6 weeks.

Before this I was at 15mg Tirz every 7 days - my dose and schedule shifted around some.

I've lost 63lbs, and I no longer require other medications for my T2DM or my blood pressure.

( I'm not looking to drop more weight ... )

I feel great !

Finding the right approach for each person is probably going to take trial and error, there are too many variables.

Perhaps a few years from now there will be real clinical testing and guidelines.

As it stands, it feels like a Roll Your Own answer.

I really appreciate you telling me about this! It means a lot to me! There are a ton of factors at play, I realize, but once I reach maintenance my plan is likely to taper to the smallest dose that still works. What a crazy journey this has been!

Appreciate it! I’ll head over and check out that study. My hunch is that if I kept taking my highest dose while maintaining, the weight loss would just continue. Not sure though — I might be mistaken about that.
 
After dropping a significant amount of weight, I started Tirz mainly to keep food noise in check. At 4.5mg, that worked well, but once the early tiredness wore off, my energy still felt somewhat below where it used to be. I had been on that amount for roughly a couple of months. I lowered it to 3.5mg and introduced 1mg of Reta, and that combination appears to hit the sweet spot. I get everything 4.5mg Tirz gave me, plus my energy is back to normal.
 
Over roughly 12 months I worked my way up to 15mg each week. Once I hit that amount, the pounds came off more slowly during the following 6 months and then stopped altogether, which is how I landed on that as my maintenance level. About 9 months later, the food noise began returning, so I ended up switching to split doses taken twice weekly (2 x 10 mg).

I’ve been considering introducing Reta, but honestly I’m a bit frightened
 
In my case, maintenance has lasted more than a year, and what matters most has been how often I inject rather than the amount. I stayed at or below 7.5 mg the whole time, since I had no wish to wipe out my appetite entirely; my aim was only to turn down the food noise so I could choose better. Over time, my whole connection with food has changed.... For the first time ever, it's a healthy one. I finally understand the joy of giving your body things you know are good for it. 😊

After reaching my goal weight, rather than lowering the dose, I raised my calories to maintenance level and spaced out my shots.

These days it's about 1 injection every 3-4 months.... My body seems to do some kind of reset.... Another 5lbs will come off with nothing different on my part.

It's almost as if it has to get used to this new weight for a while. I'm considering going down to 5mg/ week while keeping the same spacing.... Because there isn't another 5 for me to lose. A problem I never imagined having.
 
I didn't begin GLP's to shed pounds — maintenance was my goal from the outset. I had already dropped 70kg and held it steady for a year, yet I was constantly hungry. A year of low-dose ozempic followed, which brought uncomfortable side effects and only a modest reduction in appetite. Then, 1 year ago, I switched to tirz 15mg. Because hunger persisted, I layered in reta 5mg, and more recently cagri 0.5mg. Even with all of that, my side effects remain milder than what 0.8mg/w of ozempic gave me. Across the past year I shed another 10 kg, and I'm now essentially at a stable weight — 55% down, from 145 to 64kg. The biggest change: I'm no longer hungry around the clock. I'm nearly at the point where I can eat when hungry without gaining, something that has never happened in my entire life. I've even relaxed my strict avoidance of high-calorific-density foods, and it hasn't thrown things off.

The way maintenance studies define stable weight strikes me as dubious. Typically it's under 5% change across 3 months. They need some cutoff, sure, but extrapolate that over a year — 5% per 3 months compounds to 20 percent — which is nowhere near a stable weight and makes some of those findings suspect. Beginning maintenance dosing from a genuine plateau is not the same as beginning it while GLP-driven weight loss is still in progress. The dose that holds at a true plateau is the more plausible maintenance dose.

From what I've come across online, a fair number of people appear to maintain on doses lower than what they used to lose weight. But that's not a representative group — they're highly motivated and far more likely to be exercising. Even in the studies, some maintain at lower doses, though most do not.

Trying lower doses for a stretch and observing what happens seems harmless enough, provided you're ready to raise doses again if needed before too much regain sets in. Still, for most people, I believe the dose that produced the weight loss will end up being the maintenance dose.

There's also a strong case, when obesity is more severe, for remaining on full maximum doses long term for disease prevention. Most people with severe obesity — and many with less severe obesity — face very high risks of diabetes and cardiovascular disease. Higher doses prevent these more effectively than lower ones, and the risks, along with any pre-existing damage, don't fully vanish even after massive weight loss. They likely also lower long-term risk for a huge range of health problems, cancers included, and higher doses are probably more effective than lower ones there too, even without proof yet.
 
I've cut back to 1.25 mg per week so I don't keep shedding pounds. At this amount, my weight has stayed within a 3-5 lb window. That said, 7.5 mg was the highest I ever went.
 
A fairly recent study strongly indicates that keeping your highest dose is the way to go for maintenance — it looked at weight coming back when people reduced their dose for maintenance. I came across a clip about it from the Downsized on YT
 
I've kept the weight off for over 6 months now. During the tail end of losing, my tirz dose was 20 mg every 5 days. For the past 5 months I've been on 12 mg every 7 days, and I just brought it down to 10 mg every 7 days since I went under my goal. Energy is great on tirz, provided I take in enough protein. One week I swapped in 2mg Reta and my resting heart rate jumped from 70 to 110. Tirz it is for me, thank you!!

For the past 6 weeks I've been taking a maintenance amount of 2mg/week of Tirz, along with ipa+cjc1295 No Dac at 350mcg each day, and over the last 5 weeks I dropped 4kg. Today marks the 7th day since my previous injection, and I still feel no hunger at all. That said, every day I also fast from 05:00 AM until 12:00PM, which I start after injecting the ipa+cjc stack. Because of this daily fasting, I believe I've grown accustomed to eating only a small amount. My stomach has shrunk inward and I'm unable to consume large portions. Right now I'd say appetite suppression depends less on Tirz. SW:102kg GW:77kg CW:73kg. SW to GW 36 weeks

My own experience runs counter to that theory. At a maintenance dose of 2mg of Tirz each week, I keep dropping weight. What they say may hold true in some cases, yet it does not apply to everyone. It probably fits people who have moved up to bigger doses. It does not fit those who never went above smaller doses for the entire ride. My tirz began at 2.5mg. After 4 weeks I went up to 5mg, and that is where I remained for 32weeks. Since 6 weeks ago I have been on 2mg for maintenance. The weight is still coming off. There were hard moments too. Still, I never thought tirz alone would get me to my goal. Some effort of your own is required. When cravings hit, I chose to ignore food rather than raise the dose, since I understood that walking away from it would only get harder later. Right now I do not have the courage to quit entirely. But I will

Adding a stack isn't something I'd recommend without a real purpose behind it, but when there's a legitimate reason, it can work well. On Tirz my heart rate climbed somewhat; it has since dropped back partially yet remains slightly elevated, though introducing Reta didn't produce any rise for me. My thinking is that if Tirz already gave you that increase, a low dose of Reta likely won't, whereas if Tirz didn't, the odds may be higher. In any case, a conservative stack probably won't bring many side effects.
 
I take Tirz 7.5mg and Reta 3mg as a split dose, on Friday and Tuesday. Lately I've been thinking about what maintenance would look like. As of today, I figure my at-home experiment keeps going. Should my weight start drifting either way, that's my cue to revisit what I eat, how much I move, and how much peptide is in my system. I'll also keep an eye on new drugs and other forms as our world changes. I never think that I will be weight control drug free.

My home science experiment will continue until I assume earth's natural ground temperature.
