On the subject of tesamorelin, I keep hitting a wall when I try to find a definitive answer. Right now I’m running reta, and I’ve seen several sources recommend pinning tesa right when you wake up instead of after dinner or before bed—the idea being that you need a fasted state, because reta slows stomach emptying so dramatically. Since insulin shuts down the gh pulse, and food may still be sitting in the stomach even 5-6 hours later, I’ve read that this would make the tesa essentially worthless. I’ve also heard that stomach emptying returns to normal speed once you’ve been on it for a while, but I can’t locate anything backing that up beyond “trust me bro”. Have you run both at the same time? If you have, how did you go about it?
Tesa works well, but the scenario you're laying out is a real constraint. Staying fully fasted is hard when you're on any GLP-1 and dosing late in the day, right before bed. That only works if dinner happens at 4pm... And with any GLP-1, depending on the individual and how fast they digest, a 5-6 hr fast delay is entirely plausible.
As for shooting both at once, yes, that's fine.
A GLP-1, though, hangs around much longer. So if you're inside that 5 to 6 day window, it makes no difference whether you pin on the same day or on separate days. If the GLP-1 were genuinely wiping out the tesa, combining them would be impossible from the start. You'd have to pick one or the other. Nobody has said that, as far as I've seen, but in my head, sure, run them together. Just don't forget about the fasting window if you go with the evening.
I moved my tesa ipa shots to the morning. On a GLP, my digestion tends to run slow anyway. When I began, though, that wasn't something I factored in. And I don't think the results looked as good. Switching to mornings seemed to bring them on quicker, I believe.
On top of that, a lot of folks talk up the sleep advantages of tesa ipa. My experience was actually the reverse. My sleep pattern got wrecked, and falling asleep became harder with so much going on for me.
Sure, a natural GH pulse occurs during overnight rest, but that doesn't mean moving to the morning wipes those pulses out entirely.
Bottom line, mornings, and that's my preference. Still, it's likely less effective than a 5 to 6 hour fasted p.m. injection. By how much, I couldn't tell you.
Hope this helps.
