Hi everyone,

I'd like to know what drives your decision to switch things up, and how you figure out the right moment to adjust your dose. Is there some kind of schedule you stick to? Or do you simply trust your gut?

Thanks so much ahead of time.
I joined to pick up exactly this kind of knowledge. Having spent a few years at the maximum tirs dose, the results just aren't what they used to be for me. So at this point I'm weighing my options for what comes next.
For the past year I've used tirz, but about four months back I hit a plateau. My current split dose is 5mg every 4 days, which works out to roughly 10 mg/week — so I'm not actually at the maximum. I've held off on going to 15 mg/week because the STEP-1 RCT reported 22.5% weight loss at 15mg versus 21.4% at 10mg, meaning higher doses bring steep diminishing returns.

Across different trials, tirz 15mg produced weight loss anywhere from 20.2% (surmount-5) up to 25.5% (redefine-4). If I switched to max-dose reta, the weight loss range would be 24.2% to 28.7%, depending on the trial (triumph-4, redefine-4, etc.). That's not a huge improvement over tirz, yet the cost rises considerably.

Adding cagrilintide is what I'm considering, because it looks the most promising. A Lancet study pitted sema 2.4mg against cagri 2.4mg and the two together; weight loss came out to 5.1%, 8.1%, and 15.6% respectively. Stacking cagri's distinct MOA onto tirz really does appear likely to deliver bigger weight loss than making the switch to reta. And I'm not buying the claims about reta having muscle recomp advantages over tirz — that's purely anecdotal and no trial backs it up.
I appreciate you sharing this! I’ve been soaking up whatever I can, and it definitely gives me a lot to think over. I was thinking about splitting my dose and lowering it slightly just to observe how my body responds. Perhaps I ought to look into Cagri.
For me, glutamine powder has been useful in keeping my sweet cravings in check. Honestly, that craving is a beast. If archaeologists studying Megalodon saw my sweet tooth, they'd try to figure out what kind of frightening creature it came from.
I'll check out glutamine powder. There was a time I enjoyed a glass of sweet NYS wine — the sweeter, the better. These days, though, I pour a glass, take 1 or 2 sips, and wish I hadn't opened the bottle. Desserts and candy, on the other hand, have been a real problem. When I first began GLPS several years back, that issue got resolved. At present, it's returned.

I'd like to know too — how does Reta compare with a plain GLP-1? I've only just begun reta, yet I'm already planning ahead: once I hit my target and decide to come off reta, how do I do it sensibly? My thought is to switch over to a GLP-1 only for maintenance... though that's probably far off, and things could look different by then. Time will tell, as always.
The way reta affects me isn't pleasant. It leaves me exhausted, and occasionally on edge as well, and on top of that it did nothing for the food noise. Everything goes into Shotsy so I can monitor where I stand with both medications.
That fatigue won't stick around forever — it's temporary.

Look it up online. In my case, it only went on for 2 weeks. There are some odd side effects that come with it, anxiety being 1 of them.

Anhedonia is another possibility.

Also, don't be shocked if a movie's ending has you in tears without any real cause.

Still, from my perspective, the trade-off is completely worth it !

I've stopped taking it, and I feel a lot better now. I spent more than 2 months on it. It didn't suit me. I was feeling sluggish.

What's your current dose?
After roughly 2 months, I titrated up to 3mg. I've since stopped taking it.
