Reached my goal weight, but had to stop Reta because of adhedonia. My max was 1mg of Reta, taken twice weekly. Anything beyond that left me violently ill. Now I want to stay on a maintenance dose of Tirz and allow its effects on inflammation and other benefits to do their thing.

So should I begin with .5mg of Tirz twice a week and intend to stay at that level as long as it keeps working?

0.5mg seems quite low. I doubt I'd begin with anything under 2mg. Typically, people start at 2.5. You can decide for yourself, but taking 2 or 2.5 weekly would be simpler. For at least the first week, observe the effects, then tweak it higher or lower.

If reta hits you that hard, starting low might actually be the smarter move.

My reaction to the starting dose was extreme — 2.5mg hit me incredibly hard. Even after 18 months, I've stayed at or below 5mg the whole time. Because of that, beginning with a smaller amount and observing how you respond seems reasonable to me. No two people are the same.

Given that you're in maintenance and hoping to steer clear of sickness, that approach makes sense.

My own starting point was 1.25, and it suited me fine.

Perhaps 1.0 would have been an even stronger choice.

Over time I moved the dose upward, never adding more than 0.5 in one step, so that side effects stayed as mild as possible.

I started tirz at 2.5 one month back and have stayed there the whole time. Less might have been enough for me, though I certainly haven't required a higher amount. The titration plan calls for going up, yet it isn't necessary. I add a low dose of Reta alongside it to get the glucagon advantages, but truthfully I could lower the tirz. 2mg seems like the right amount. Still, if you're extremely sensitive, trying a smaller amount can't do any harm.

For roughly 7 days, I'd begin by swapping out about 0.25mg of reta in favor of tirz. After that, 0.5mg, followed by .75mg. After you're completely switched over to tirz, the dose can then be fine-tuned.

Since I've already stopped taking Reta, there's no reason to bring it back. I plan to move straight over to Tirz, and I'm hoping the same side effect won't show up.

Congrats on reaching your goal weight with such a tiny amount of Reta.

I’ve gone from Tirz over to Reta before, and I figure at some stage I’ll go back the other way to Tirz.

Since you’re presumably using tirz to keep appetite and food noise in check: keep in mind that the injection needs roughly 24 hours to hit peak levels (Tmax). Start with that .5mg plan you mentioned, stay on it, and observe how you respond. When food noise or appetite starts creeping past what you’re comfortable with, that’s your cue for another dose. Log what you take and when, and let your appetite dictate the pace rather than the calendar or a fixed schedule. Once you’ve figured out what you need, build a schedule from there.

In my case, food noise returns before any real hunger does (hunger was never my problem—it was that “I could eat” chatter). While bridging, whenever I caught myself thinking about food, I’d add a bump of Tirz to quiet the noise while Reta worked up to a therapeutic level… it took two pins. I’d also dose before a known “stressor” to give myself extra capacity, though it turned out not to be an issue.

A coworker of mine gave Tirz a shot for the first time this week, at 2.5mg. That same night he felt awful, and heartburn had him awake the whole night. By day 3 he was back to feeling normal. No 2 people respond the same way. Try starting smaller and see how it goes. The only way to find out is to attempt it. Wishing you the best.

Update. I’ve now taken my third .5mg Tirz shot, dosing twice per week, and my eating hasn’t decreased at all. The only thing I notice is nasty burps the day after injecting, and past that it feels like nothing was administered. Starting tomorrow I’ll move up to .75mg twice a week.

I’m trying not to push past the right level, and because of the half-life I realize what I took on Sunday is somewhat stacking up.

On Reta, the flat feeling was too much for me, yet I sort of miss how quick and brutal it hit. My metabolism felt completely conquered. 😂 Still, if I give it some time, and considering that so far Tirz has me feeling pretty much normal, this could end up being the better option.

Update: 1mg twice a week is where I am, and the Tirz isn’t doing its job. I ate a whole dinner of burger and fries, followed by a giant cookie and a glass of milk. That cookie, though, is really making me feel ill.

I miss having that take-it-or-leave-it attitude toward junk food.

For my next dose, I’m moving up to the standard 2mg.

Possibly this suggestion comes too late, though one more route would be to simply lower your reta dosage and check whether that eases the anhedonia.
