First Tirz Dose (2.5mg) Taken Last Night

Kicked off my tirz journey at midnight yesterday, using a single-dose disposable pen.

Some background on me:

Height: 183cm (6'0")

Weight: 97 kilo (~213lbs)

A1C: 6.0

BP: 150/90 (no meds)

RHR: 42bpm

It took a solid 2 or 3 minutes before I could bring myself to press the trigger. Barely felt a thing, so it should be simpler next time. The injection went in 3 inches to the right of my belly button.

The instructions say to keep the button pressed for 10 seconds, but with the 2.5mg pen it feels like the injection finishes way quicker - more like 2 or 3 seconds? I kept holding the button down for what felt like far longer than the actual injection took, or am I wrong about that?

Went straight to sleep afterward. 12 hours have passed now and I notice no difference. No nausea, no stomach pains, etc. Can't tell yet whether my appetite is affected - I had a light breakfast and only a chicken breast for lunch. Not hungry, but I could eat if I chose to. Does it need a few days to build up in the system?

I've been careful to drink water every few hours. Any other tips would be appreciated.
 
Yeah, with these pens the full dose is frequently released long before you reach the end of the hold time, so it's not unusual for it to feel like only a couple of seconds passed — provided you kept pressing the pen down for the whole ten seconds as the booklet instructs. At 2.5, any change in appetite tends to be quite mild, or it may take anywhere from a few days up to a week before you really pick up on it, so being fine 12 hours in is a reasonable beginning. The biggest thing I took from my own run: keep your protein intake high, don't push yourself into large meals when hunger isn't there, and let the first couple of weeks play out before you expect the scale to move much.
 
Great!...What's your goal for how much you want to drop?

In my case, 'injection day' doesn't bring much at all in terms of sensations...Around day 2 & 3, stomach gurgling shows up along with a feeling in my throat resembling acid reflux....that's when it hits its high point, then it continues for another day or two before fading out around day 5 or 6....

At 2.5mg the dose is quite low, so whether your reactions end up milder than that is uncertain...still, the important thing is you've begun....good luck... 👍
 
I hadn't anticipated that. Pain was basically absent, likely because the needles are extremely fine.

Peak blood concentration arrives roughly 24 hours following an injection, and how each person reacts to these medications varies a great deal — whether that's a favorable reaction such as reduced appetite or an unpleasant one such as nausea. That means you might feel nothing initially, and if something does show up, the following day is when it's most probable. One useful thing to keep in mind: because the half life is around a week, levels accumulate in your body across 4 weeks following a dose increase or when you begin, which is exactly why 4 weeks are waited between increases.

Given all that, predicting your response is really difficult. In the first few weeks, the most probable outcomes are slightly lower hunger, filling up somewhat more easily, or a touch of nausea. Still, no effect at all, or severe gastrointestinal side effects, can happen even if they're less likely. Unless side effects appear or your current dose is already doing the job well enough, simply stick with the standard dosing schedule for increases. I don't know which dose you're targeting or how much weight you hope to drop, but that also plays into when doses go up.

During my university years I was on both the soccer and basketball teams, and all that running kept my resting heart rate low. Back in college it sat at 35 bpm!

I noticed as well that there's still some liquid remaining in the pen. It doesn't seem like much, though possibly around 10% of what it started with? Is this expected, or did I do something wrong when injecting?

Over roughly 5-6 months on tirz, my weight dropped from 224 down to 184... the pace was slow and steady, about 2.5 lbs per month... the majority of the loss came during the first 2-3 months, and then it eased off over the final month or two... for me it was a really comfortable decline... nothing at all like past attempts at weight loss... my hunger was lower... food just didn't hold much appeal... whenever a bit of food noise showed up, a small handful of nuts was more than enough and kept me satisfied for a few hours... at meals I'd finish less than half of what I used to eat and save the remainder for a later 'meal'.....

Through all of it, everything described above felt totally normal and came without any struggle...

My schedule was 2.5mg for 4 weeks, then 5mg... then 7.5mg....

For me, tirz has been a monster. I dropped from 293 down to 178. That's close to 40% loss, and the number keeps going down. Back at the start, I remember the full effects hitting me within 2 days. Good luck to you!!

Through week 15 I have for the most part stayed at 5mg with the tirz, and here is what the past week was like: by injection day the appetite suppression had vanished and the food noise had returned, and that carried over into the following 2 days, after which things get better. This week the effect feels like it is fading, yet I chose to remain at 5mg at least from week 15 to week 22, purely to see whether the weight keeps coming off. Should I stall or slide backward, then I will think about either pinning 5mg every 4 or 5 days or moving the dose up to 7.5mg.

What this really is, is a test of restraint and self control, given that I have both tirz and reta on hand to experiment with, and I have already made questionable impulsive switches out of FOMO and the wish for quicker weight loss. Still, so long as I am not putting weight back on without much effort, I have to remind myself that it is working, and that sometimes it is simply a plateau.

Great!...When it comes to how I titrate — especially once I switched from the pre-measured compounded pharmacy doses to kits — the kits give me the ability to dial the dosing in myself...I'll keep raising my dose until the burping picks up a little, there's some rumbling in my stomach, my stomach feels kind of full pretty much around the clock...and I get that throat sensation like acid reflux is just about to hit.......but I won't go beyond that point

A fixed mg amount isn't what I aim for, and neither is the dose or titration that 'test studies' suggest...I go up until those 'symptoms' above show up...that's how I know it's working....

Honestly, that approach makes a lot of sense in practice. The key part is figuring out your own response and being able to play around with it — start at a given dose, then tweak until it suits you... dosing that's customized to the individual.
 
Instructions say to hold button down for 10 seconds, but it seems the 2.5mg pen injects much faster than that - like in 2 or 3 seconds? I was holding button down for what seemed like much longer than the shot took to fully inject, or am I mistaken?

Those 10 seconds aren't there to push the stuff in — they're there to keep it from coming back out (of the body).

That matches what I intend to do. Over the next six months I'm aiming to spend 1 month at 2.5, then 4 months at 5mg, followed by 1 month at 7.5. Was your increase this gradual? I wanted to play it safe.
