Tirze to Reta ratio

Hi,

I've often read that some people combine tirzepatide and retatrutide to achieve greater appetite suppression ("food noise" reduction) along with improved fat loss.

I've also noticed that some gray-market vendors sell blended formulations with a 2:1 tirzepatide-to-retatrutide (T) ratio. However, I have no idea where this ratio comes from or whether it has any scientific basis.

Is there any evidence supporting this specific ratio? Has anyone here experimented with different T ratios, and if so, what was your experience?

I’ve not seen that blend with suppliers before. I’ve been taking Reta on a Monday and supplementing with Sema or Tirz on a Thursday, so the blend wouldn’t work for me in that way. Reta has less appetite suppression than Tirz, so I suppose it makes sense other to be a blend. I believe EL (the makers of Reta) are doing clinical trials on the effects of combining Reta with Tirz now.

Tirz is definitely the gold standard for suppressing food noise, and I did find people saying that they were really struggling with that after switching from tirz to reta, so I wonder if it could be a way to counter that?

I'm on 15mg tirz, 3mg reta (so a 5:1 ratio) and 0.2mg cagri. I had been stalled for 5 months on 15mg tirz alone which is what drove me to try stacking. It's been very effective, but I don't think I would blend the two, because I like having more control over my doses.

Blending the two peptides is certainly not without risks. I would never mix them in the same syringe.

Even when administered as separate injections, I suspect there may be an optimal dosing ratio between the two. That ratio may vary from person to person, depending on individual response.
 
There is no standard ratio. This highly depends on individual preferences.

My GLP weekly schedule is 8mg Tirzepatide + 6mg Retatrutide split up this way:

Monday evening & Friday morning > 4mg Tirz

Wednesday evening & Sunday morning > 3mg Reta

That's almost a 2:1 ratio.

How did you titrate each peptide? What dosing schedule did you follow?

I was on 7.5mg Mounjaro before I started adding Retatrutide (and substituting Mounjaro with grey Tirzepatide.

Adding Reta:

2 weeks @ 1mg

4 weeks @ 2 mg

3mg after that.

I tried 8mg Reta/6mg Tirz for a few weeks but I prefer the other way around for better appetite suppression.

For sure. The research I did before I started experimenting myself seems to show people are fairly split on whether they take more reta or more tirz, but if the info about an official trial by EL is true, I'm very interested to see what their results show.

For me, I was on Mounjaro for 12 months, starting as part of a medical study. At around the 12 month mark I hit a plateau and despite trying everything I could to break it for five months I couldn't shift the scale. Which would be fine if I wasn't still medically overweight.

So I switched to grey tirz after 17 months, as well as low doses of reta and cagri. I'm now 75kg and losing 0.5-1kg a week which is perfect so I'm planning to stick to this as long as possible. If I hit another plateau, I'm planning to increase to 4mg of reta. Anything more than that and I'll lower the tirz when increasing the reta.

You added retatrutide only after you'd been at a weight-loss plateau for about five months and had already reached the maximum recommended dose of tirzepatide (according to Eli Lilly's dosing guidelines).

That seems like a reasonable strategy. However, I'm considering a different approach by introducing retatrutide much more gradually. My tentative plan would look something like this:

  • Step 1: Tirzepatide 2.5 mg/week for at least one month (I may extend this to five weeks or longer, depending on side effects and my rate of weight loss).
  • Step 2: Continue tirzepatide at 2.5 mg/week and add retatrutide 1 mg/week, administered as a separate injection, either on the same day or on a different day of the week.
  • Subsequent steps: Increase only one peptide at a time—first tirzepatide, then retatrutide—following a slow and cautious titration schedule.
Of course, this is only my personal idea and not something based on clinical evidence.

I just also completed a four-week washout period because I absolutely needed to resolve the persistent diarrhea, which had been present for the previous six months, and I want to start again using the same initial dosage I used 1 year ago..

Yeah I am extremely fortunate that I responded very well to the tirz for a year before I plateaued, without any side effects. I imagine it must be really difficult to figure out titration when you're struggling with GI issues.

Hopefully you do better on reta and don't have such bad issues this time around!

I used Tirze for 6 months about, then I moved to reta. In both case when I reached 6mg/W (Tirze) and 5mg/W (Reta), diarrhea increased a lot and I had to move back to a lower dosage.

I hope that the new strategy will work better.

Yeah, a five month stall would be really discouraging. It's good to hear that your strategy is working. I've been doing 2 mg reta twice weekly for a few months but find food noise is returning lately so I've been thinking of replacng one of those pins with an equally small dose of tirz.

7 weeks. I've gone from 80.2kg to 75.8kg so far.

I had been bouncing between 78.9kg and 80.5kg for the five months I was stuck before that.

Honestly I'm thrilled. I could possibly have tried just the cagri alone because I think some of my problem was suppression wearing off majorly on the couple of days before I pin my tirz, but I was impatient and frustrated, and because it has been so effective for me I don't want to mess around with the stack.

I have found I am able to focus a lot more on making the things I actually eat count. I have been really careful with my macros which was proving really difficult on tirz alone.

8:6=1.33:1, closer to 1:1 than 2:1.

I’m fairly sure that’s an example of you trying to conform observations to your way of thinking… called confirmation bias.

in general people tend to add what they think they need to what they’re doing to achieve whatever combination of effects. Little science is involved, people are going off vibes and perceptions.

I take 20mg of Reta and .25-.375mg of Sema. I’ve thought about switching Sema to Tirz. I expect I’d need 2.5-5mg/week of Tirz to get the same outcome. That would be a 1:4 ratio at best.

Defnitely, you should be thrilled. Those 5 months would have been really discouraging, so the return to shedding weight at that pace must feel great. Are you still far from your goal weight? Keep at it - you're on the right track!

Thank you! I'm definitely relieved. And will be following this thread closely to see what other people say as I'm particularly interested in this.

68.9kg puts me into the healthy BMI range for my height, so I'm hoping to get to 60kg and then maintain so I have room if my weight ever creeps up again. I started at 116kg so I have a way to go, but I've done most of the hard work for sure!

Good luck with your switch if you decide to make it. I think it's a great combination, personally.
 
I'm currently doing something close to a 4:2:1 ratio of Tirz/Reta/Cagri.

15mg/6D Tirz; 6mg/6D Reta; 3mg/6D Cagri. Each staggered 2D from the previous.

I wouldn't dare go any higher on the Reta, as the known side effects are beginning to hit me pretty hard. I wake up after 4 hrs sleep. Luckily I can watch some mindless TV for an hour or so and then fall back asleep easily enough, and get another 2 hrs sleep. And, ahem, the, uh, "gastrointestinal distress"...

On 10mg Tirz I plateaued at 40lbs lost. I tried going higher but the allodynia, dysesthesia and tinnitus made me pull back. I added the Reta and Cagri to address the stall, at a lower dose of 2 then 3 Reta and 1 then 2 Cagri; dropped an additional 16-20 lbs, and plateaued again.

Raising to the current dosage has me moving again, but I also cut out the protein shake I was taking thru the course of the day to curb the GERD I was getting. Instead I bought a product called RefluxRaft (an Alginate/bicarbonate gel that coats and neutralizes stomach acid). I was making the shakes with whole milk, and they were costing me 400 calories a day. Raft is 12.

2:1 is just a starting point, not a final statement and it is what I found out in gray market blends.

Of course if you are having good result with only one peptide, there will be no reason to add a second one.

But in case you aim to use both since the beginning which could be the best combination?

I'm on this schedule for about 3 months and it's still working. The last 6 weeks I still lost 2.5kg (5.5lbs).

That is despite a nearly 3 week holiday in France and Spain. I haven't exactly been gorging at buffets, but I can definitely appreciate the mediterranean cuisine 😋

The current dosage permits me to enjoy food in "normal" quantities but gives enough suppression to stop eating when I should. As long as I stick to healthy choices, the scale continues to drop.
