Hi everyone,

I'd really appreciate hearing feedback and stories about how Tirz and dosing have gone for other people.

I'm on the verge of beginning week 3 of tirz, and I've never used any GLPs before. My dose is 2.5mg (1 injection each week), and the side effects have been quite mild — occasionally I feel nauseous or get acid reflex, typically the day after my injection, but it's manageable and I simply use over the counter medication.

I'm getting close to 4 weeks, which I've read is the point where a lot of people increase to 5mg. Yet my appetite remains very suppressed and I'm truly not eating the way I used to. I definitely don't want to eat more, and all the food noise has definitely quietened. I just haven't noticed any weight loss. I exercise regularly.

Is it too early to be losing substantial weight? And also, should I dose up even if I think 2.5mg right now is still ok?

I'd love to hear what others experiences have been and if anyone has gone through the same

Thanks

I began at 2.5mg of Tirz, kept that dose for 4 weeks, and the suppression I got was enough. That sounds close to your situation. But my plan was to stack it with Reta, since I intend to raise that one for the Glucagon advantages, while still wanting Tirz to quiet the food noise. So I introduced 1mg of Reta. Once both were on board, the drop in food noise was another big jump.

So the point is, Tirz did not have to be increased in my case. I simply wanted to begin titrating the Reta. For now, I figure I will stay at this level for a while. My Tirz start date was March 30, and at this point I am down 20 lbs. A lot of that is water, I realize, but some fat is clearly coming off too.

On top of that, I count calories (averaging a 1000 calorie deficit), log at least 5 miles of walking daily, and lift weights 3-4 times each week. The GLP medications give me the headspace to actually run the deficit that moves things forward. If you log every last small calorie, down to a ketchup packet you happen to smell, you get the hard data showing why the scale is or is not moving.

If your appetite is already pretty low and 2.5mg of tirz has you feeling queasy, then jumping to 5mg is very likely to make that nausea a lot worse. It is somewhat difficult to judge from what you wrote just how severe things are right now, which makes it harder to say how a higher dose would affect you.

I have no idea whether your goal is dropping a small amount of weight or a very large amount. If it is a lot, then at some point you will certainly have to push the dose up, but nothing says you have to rush it. One path is to just try 5mg and see what happens; if you end up vomiting, that tells you it is too much, so you drop back to 2.5 and hold there before climbing again much more gradually, or you nudge it up only slightly to 3 or 3.5 mg. When it comes to figuring out whether a higher dose will cause trouble, side effects tend to be the most reliable signal.

For me the pounds came off quickly right at the start, then things inevitably leveled off, which makes this a strange spot to be in. My PCP is a bit unconventional, so I ended up following a pretty forceful titration plan with tirz while I was at the heaviest I have ever been. That said, 4+ pounds a week is something others achieve too — on only 1 mg of reta, in fact. It could also be that you are experiencing body recomp (losing fat while gaining muscle) or other victories that do not show up on the scale.

Grey offers so many choices that it is easy to drive yourself crazy. Keeping to the program tends to be one way to remain sane (or to end up with more side effects). Per se, tirz under 5 mg per week is not viewed as a therapeutic dose. Many people do microdose, though, such as when GLPs are used only for optimization or a small amount of weight loss.

Side effects and my weight loss usually arrive together, so to me they are not automatically a bad thing. Besides Zofran, I keep various OTCs on hand in case I need them, although for some rough stretches they have helped only so much. Generic Voquezna from India was what I used for heartburn. When I remember, I also take probiotics, and on top of that I sometimes drink Kefir or eat sauerkraut.

Building on what @lessthanhalf said, I would think about roughly 2+ mg of tirz twice weekly (raising the dose while keeping sides in check). Or I would even think about adding reta or switching to it now, as @msyntakz brought up. Tirz is something I love and I have a large stash of it. Still, reta is what most of my peptide friends use, and these days reta is the only GLP I purchase to stockpile.

Early on, when I ran into that very same bind of my own making (and it still crops up now and then), it helps me to keep in mind that this isn't a race. 🙂 (Unless it is for you, though I sort of doubt that).

I'm fairly confident that most of us would agree on this: keeping the dose lower and the stack simpler (or skipping a stack altogether) for as long as it stays workable and effective is the better path for your overall well-being.

Staying at the lowest dose that still lets you lose weight also costs less.

Putting real attention on diet, exercise, and good sleep (genuinely) is, in my view, a basic and essential companion to the meds.

Hi!

I'm just about done with my 5th week at 2.5mg. I'd thought about going up, but given how well it's going and how strong the appetite suppression is, I'm holding off for now.

If anything, this 5th week feels stronger than week 3 did, which was probably the point where I noticed the least. The sides (mostly nausea and tiredness) have eased off at the same time. Down 5kg so far, and exercising consistently. Feels like it's working, and until that changes I'm not touching anything 🙂

I bumped my dose from 2.5 up to 3.75 once 4 weeks had passed, even though I was still dealing with a bit of nausea. Bad idea! Give the 2.5 more time to accumulate in your body over another few weeks, and after that make only a modest step up rather than going right to 5. For those who tend to get side effects easily, a higher dose simply makes them worse.

My experience with Tirz has been excellent, and I've now been using it for 1 year. For calories, I aimed at tdee-500cal, which put me around 1500. Going any lower than that number left me with no energy at all. Rather than sticking to a fixed schedule, I moved my dose up whenever the food noise started coming back. The starting dose was 2.5, and for the past 7-8 months I've stayed at 10. Weight loss kicked in almost right away for me. What I'd strongly recommend is logging what you eat and raising your dose once the food noise begins to return.

The whole point of dosing these meds on a monthly increase schedule comes down to people being dumb — if the guideline said 5 or 6 weeks, they'd screw it up constantly. 1 month is simply easier to keep track of

(yes that's only my guess. But I think I'm right).

So my suggestion is to do whatever you think is right for yourself. And generally increasing the dosage when the med is working is NOT a good idea.

Especially because at week 4, you might not have reached the max levels in your body... so zero reasons to double it now.

As other said, a better move is titrate up slowly as soon as you feel the appetite suppression fading

Thanks — this is genuinely useful, and it lines up with what I was already leaning toward in the back of my mind.
