Tirz quit on me and Reta seems to boost my appetite — advice (or pizza 🍕) welcome

I began with Tirz and within 3–4 weeks had to scale back to tiny doses, since the effect hit me very hard and the side effects became too much to tolerate. After that I moved up in very small steps until I got to 2.8 mg doses. My guess is it accumulated in my body, because after three doses at 2.8 mg (2.8, not 28 🤣) I felt genuinely awful. Since my doctor couldn't determine whether Tirz or my other health problems were to blame, he told me to pause for 3 weeks.

Once I restarted, the side effects were completely gone — but so was any benefit; I never felt full at all. At the moment I'm raising Tirz every 2–3 weeks and have added 0.8 mg of Reta, yet Reta appears to be making me hungrier. So now I'm stuck deciding whether to go up on Reta or on Tirz. Currently I'm on 5 mg Tirz plus 0.8 mg Reta.

Of course none of us are alike and each body reacts its own way, but I'd love to hear from people with more experience, or anyone in a similar spot!

Any thoughts or suggestions — or is trial and error just the reality at this point?

Not long ago I stacked Cagri on top of my max Tirz dose. Appetite control was fantastic, but a mild allergic reaction showed up, so I quit it after 2 doses. Since then I've been stuck fighting a stall that just won't budge.

4 weeks back I introduced Sema at the lowest dose. The appetite suppression hit hard — strong enough that I have to push myself to eat enough calories daily — yet the scale number hasn't moved. Aaggh! Still, I'm heading into month 2 with it, so this week I'm lowering the Tirz slightly while raising the Sema. 🤞

I can't recall exactly who, but I've read several posts here from people who added sema and finally broke a long stall, so maybe a few of them will jump in. Cagri is clearly a common add-on as well, plus the newcomer Elora, and those recent Elora/Tirz combo studies look pretty damn impressive.

Thanks a lot! 🫶 Cagri and Elora are both still untested for me, and honestly, Sema never even crossed my mind as an add-on — that's actually a really good idea! I'm curious to hear how month 2 treats you with it. Fingers crossed for you! 🤞

Truth be told, there's Reta sitting in my freezer and Elora headed my way, but I deliberately started with the least expensive choice. Why? Because I'm cheap. 🤣 My thinking was that if the cheapest one gets the job done, I come out ahead. Tirz is something I plan to stay on for life—I want every health benefit that comes with it—but I still have some weight left to lose.

Maybe the Tirz wasn't what was causing those side effects after all?

Titrating every 2-3 weeks means it could take some time before you're back at higher levels.

My suggestion would be to stay at 5 mg for 4 weeks before moving up, and only increase if the stall continues.

Hydration matters a lot too!

Be sure you're getting plenty of water.

HAHA!! "Send Help (or snacks!!..." is a great line.

You're in Italy, so you've also got access to some of the BEST FOOD anywhere! (Carbs and pizza are my weakness — that's why my avatar is the Michelin Man.)

Honestly though, how far you can push depends on where your supply comes from — maybe just keep dosing until you hit the wall?? 😅😅

I was @ 9 mg of Tirz and dropped back to 7 mg since the difference really wasn't noticeable.. No sides, but nothing like a sledge hammer effect either...

Obviously, YMMV

You should check a graph showing your blood levels derived from the half-life of the GLP-1 medication, factoring in how much you injected and on which dates.

It takes roughly 5 weeks for a GLP-1 to reach full buildup, and another 5 weeks to fully stabilize following any dose change.

Confused? On the day you stopped, the GLP-1 amount in your blood was much higher than what you currently have at week 3 of restarting the GLP-1.

Put simply, your system currently holds less GLP-1. Rebuilding it takes many weeks.

Reta gave me neither appetite suppression nor weight loss until I reached 6/8 mgs. My own advice would be to pick one or the other, and if you choose Reta, go higher!

Anyone who says even slightly negative things about reta while taking under 2 mg gets an automatic spot in the reta bro/family hall of shame.

For those stacking, a simple route to 2 mg is 1 mg twice per week.

Six months on Reta, and I'd worked my way up to 12mg/week — yet my food noise never quieted down at all....

Honestly, it felt like I was STARVING the whole time. Every side effect showed up: awful reflux, constipation, what felt close to gastroparesis, reflux keeping me awake at night, sulphur burps — and still a hunger I couldn't satisfy!

Then I moved to Survodutide plus Tesamorelin — eleven months in, I'm down almost 30 pounds.

That's interesting, since I've been thinking about going the budget Reta route once more. But did you run survo on its own, with no tirz? And what survo dose were you using?

Honestly, same — pinching pennies is just being smart! 🤣 If the cheaper option gets the job done, why spend extra? And I'm with you on Tirz: the broader health perks are way too valuable to walk away from down the road. Fingers crossed Sema finally busts through that plateau you've been stuck on!

I really appreciate the suggestions! The lab results didn't give us a clear answer, so the cause was never confirmed — but thankfully things improved on their own over time.

This time I'll let it accumulate in my body before expecting anything, rather than pushing the pace. Also, thanks for flagging hydration — I should be more careful about getting enough water in! 🙏

Haha, thanks! You're absolutely right — being surrounded by incredible food in Italy is its own kind of torture when you're trying to get appetite suppression figured out! 🍕🍝

Hearing that 7 mg and 9 mg felt about the same for you is genuinely interesting. Dosing until I “hit the wall” is a tempting idea, lol.

Good to know YMMV really does apply this much here. Thanks for the insight and the laugh! 🫶

I appreciate the explanation about blood levels and half-life! To be clear, I wasn't expecting the 5 mg to hit the same way immediately after that 3 week pause — I actually began again at very low doses and worked my way up slowly across several weeks.

So I fully understand that my blood levels are still climbing toward steady state! What caught me off guard was simply how differently my body's tolerance and appetite response felt this time versus the first. Thanks for breaking it down!

At low doses, Reta ramps up my hunger. Cargi and/or Sema should hit the brakes fast.
Thanks! Hearing that someone else has hit the same hunger spike on low-dose Reta really puts my mind at ease. It's also great to know that Cagri or Sema work so well as brakes — I'll certainly bear that in mind as an option!

I appreciate you sharing that. It really clicks with me — 0.8 mg is a very low Reta dose, so hearing that things changed at higher doses is helpful. Keeping to a single peptide at a time sounds like the smartest approach too. Thanks for the guidance!

Haha, hold off on putting me in the Reta Hall of Shame for now! 😅 Honestly, I'm jealous of anyone who gets appetite suppression from Reta — at 0.8 mg I'm just sitting here hungry!

Good to know that split dosing can get you to 2 mg; that really does sound a lot more manageable. Thank you so much! 🫶

That sounds truly awful! I'm really sorry you went through all those horrible side effects only to discover Reta wasn't the right fit for you.

Still, well done on breaking through that plateau with Survo and Tesa! Dropping nearly 30 lbs after everything you endured is fantastic progress! Appreciate you sharing 🫶
