Week 41 of my Mounjaro journey, and I've dropped 63lbs in that stretch starting from 350. I did almost 5 months on 2.5, a month on 5mg, two months on 7.5, and this weekend I finally moved up to 10mg. Yes, it's only been 3 days since the 10, but today I'm ravenous — I'm eating 2200 cal a day, yet stick a pizza in front of me right now and I'd demolish it.

Honestly, that satisfied feeling has been gone since 2.5. I'm after advice and guidance here, because I'm seriously thinking about finding something to add that would quiet the food noise and the hunger. If things keep going like this, I'll eat my way straight through 10, and I'm worried the best of it is behind me and the next two mg won't fix anything.

I know it's a silly question, but I had 2 5mg pens left so that's what I took — could it be they just weren't potent enough, or should I give 10 a full month?

Right now I'm going to bed hungry.

Have you considered a different injection site? From what I've heard, if the belly is your usual spot, trying the arm or glutes may give things a positive boost. I'm on Tirzepatide myself and my weight loss has been flat for a few weeks now, so before I increase my dose I'm going to give my arm a try for a while. As for the glutes, they did NOT work for me at all.

Since you've been using it for quite a while now, why not try 12.5 and observe the results?
It's been just 4 days since I started 10, so I need to give it at least a month. My insurance will only cover refills every 25 days.

Depending on how old the pens are, they may have lost some potency.

Still, it probably isn't much.

4 days isn't long enough to judge well; levels will keep building in your system for 4 weeks.

For pre-GLP1 days, that rate of loss would have been seen as average given where you started — it only looks almost slow by today's standards.

In my view you've dropped a solid amount of weight, and it may be time to re-establish your set point. By that I mean it should be fine to bring calories up a bit (you held quite a high deficit for a long stretch), and perhaps work in some low impact cardio.

Once hunger is no longer an issue, you can do another cut. No need to stress about maxing out Tirz — several people on this forum have gone well beyond 12.5mg, and more agonists are available now or will be soon for stacking if that becomes necessary

Would you consider stacking? I use 10mg Tirz and began stacking .25mg Cagri (4 days after my Tirz injection). I've also read that some people do well by adding Sema instead of Cagri.

I'm on week 41 of my Mounjaro journey down 63lbs on that time from 350. Stayed on 2.5 for almost 5 months, 5mg for a month, 7.5 two months and just went to 10mg this weekend. I know its only been 3 days since I had the 10, but I'm starving today, I am doing 2200 cal a day, but if you put a pizza in front of me right now I'd devour it.

I haven't had the satisfied feeling really since 2.5. I'm look for advice and guidance, I'm seriously considering finding something to add to stop the food noise and hungry. If it stays this way I'll eat myself through 10, and i'm worried I've gone through the best of it, and the next two mg wont fix anything.

After injecting tirz, peak levels usually show up somewhere between 8 and 24 hours, so by now your blood concentration has already topped out. What you're going through is pretty typical — I went through the same thing, except I never allowed myself to feel hungry, I simply hit a plateau. The reason is that the more weight you drop, the stronger the hunger signals become. On top of that, each step up in dose gives you less added appetite suppression (going from 0 to 2.5mg produces a bigger suppression jump than 7.5 to 10, even though both are 2.5 increments).

For me, the fix was adding a small 3mg dose of eloralintide, and it changed everything. It let me break through the plateau I'd been stuck on and keep shedding the last few pounds. If appetite suppression is what you're after, that's your best next move.

Alternatively, you could swap 10mg of tirz for 10mg of reta, and you should keep losing weight over time, because reta won't give you a dramatically stronger appetite suppression right away, but its glucagon effect will boost weight loss.

In your position, elora would be my pick, and it's getting easier and easier to find, though landing a good supplier is sometimes hit or miss.

Good luck!

Because the half life is 6 days, levels keep climbing while you stay on the same dose for as long as 5 weeks, and they don't peak until the 5th.

A useful tool for tracking your levels:

I agree that's accurate, and reaching peak effect does generally require several half lives. That said, based on what I've seen myself and even running the numbers, my take is this: if the concentration after one shot doesn't move the needle at least somewhat, then whatever extra he gains from stacking half lives of a 2.5mg step-up will probably be small and probably won't deliver enough appetite suppression. Adding eloralintide would beat all of that by a wide margin. I have 0 doubt about it.

Just my experience and 2 cents.

Would you consider adding a small amount of Cagri — starting at .25mg — on the day following your usual GLP?

Of the 41 shots I've done, 38 went into my belly, 2 into my arm, and 1 into my thigh. On Saturday I may put the 10 in my arm just to see whether anything feels different.

Those had sat in the fridge for about 77 days. I realize it might have lost some potency, though not enough to really matter.

I'll stick with it for a month, but judging by the first 4 days, this month is going to be a tough one.

Sure, I'm willing to give that a go — I just need to figure out what to pair it with. Reta is something I'd rather avoid; with my AFIB, I don't want to risk anything that could mess with my heart rate.

Our timelines and titration pace are pretty similar. I'm at week 42 and still on 7.5. Around 2 weeks back I hit a stall and felt hungrier, so instead of every 7 days I now take 7.5 every 6 days, and that seems to have made a difference. Perhaps an earlier dose rather than another increase is worth a shot to see if it helps? I figure we're all just working out what suits us along the way. Good luck!

I'll try that this week and see how it goes (pun intended). Figured I might as well use my arm too, just for the fun of it.

Have you thought about splitting your dose? Some people find that helps them avoid getting hungry toward the end of the week before it's time for the next one. Another option could be adding high volume, low calorie foods to help you stay full. I hope you find the approach that suits you best!

My mistake — what I meant was that you've been taking the drug for a long enough time, not that your dose is high. I don't have any experience with insurance, so I can't say how you'd need to handle that side of things. That said, if insurance is what's blocking a dose increase, you might simply accept a plateau for now until more drugs, or different drugs, become available to you. In the meantime, aim to eat at maintenance so hunger isn't something you're battling.

That's interesting. I found that tirz injected into my belly wasn't producing any suppression, so I put 15mg in my delt and ended up with no appetite for about 3 days....
