Stacking Tirz & Reta: what's your long-term plan?

Tirz has been my medication for 18mos (sema came before that, for a year), and I've dropped 80lbs so far with another 30lbs left to lose. My Tirz dose had reached 15mg, so 10 weeks ago I began adding Reta with the intention of eventually switching over. Since Tirz felt like it had maxed out for me, I figured trying something different was worth a shot. In those 10 weeks I've lost 3lbs.

Right now I'm running 10mg tirz alongside 4mg reta. This week the plan is 7.5 tirz plus 8 mg reta. Keeping up with food logging for more than a week is proving hard for me. The main reason is that after 30 years of prior tracking, I effing hate doing it.

With the reta increase, I'm hoping a calorie deficit will feel a bit more manageable, but it leaves me wondering - how long will I keep stacking these, and does it even make much sense? When should tirz be dropped completely, or is that even the right move?

My current stack is 5mg Tirz, 5mg Reta, and 1.5mg Cagri. There's still some noise, but it's manageable, and I'm able to get my calories and nutrients in. I'm no longer in a deficit—right now I'm just focused on building muscle mass—but so far this combination is working well for me. I've been stacking Tirz with Reta for roughly 8 weeks (4 of those with Cagri). I began at 15mg Tirz and lowered it each week while raising Reta each week. I knew I wanted at least 4mg Reta to get solid glucagon effects.

If the main goal were keeping hunger and food noise in check once high-dose tirze/ reta started losing its punch, I'd lean toward adding an amylin agonist such as cagri or elora instead.

That said, having at least a rough sense of your calorie intake really is essential for holding a steady deficit. After a while it ought to become second nature anyway, since the food combinations you eat regularly are fairly limited.

For maintenance, tirz serves as my foundation, and I bring reta in on a rotating basis whenever I feel the need. It also took some time for reta to build up in my system, but once it did, it melted away my final 20 like butter

At what dose did it finally kick in for you?

Cagri was a bust for me — 8 weeks on it and the only thing I got out of it was feeling wiped out.

Tracking? Maybe once a month, if that. It gets old fast since my meals barely change from one day to the next.

I still have 30lbs to go before reaching my goal weight. For now, my plan is to stay on Tirz (plus Elo if needed) for maintenance, and to add some Reta in case I need to shed a few lbs.

A year from now, who knows what the newest weightloss pep will be 🧐

For now, yes — beyond weight loss, both of them deliver extra health benefits as things stand. If other options emerge later on, I’ll likely give those a try as well.

cagri hit me the same way. The tiredness got to be too much, so these days it only makes an appearance every now and then

Adding muscle is the step most people leave out, and they end up regretting it 12 months down the line. Smart choice

Right now I take 7.5mg of tirz weekly. Rather than moving up to 10mg on the next step, I've been considering adding reta instead of raising tirz further, so I can start getting the glucagon benefits. A doctor I came across said the glucagon effect begins at 4mg of reta. My idea was to halt the tirz up-titration and work toward the 4mg reta dose, assuming I can tolerate it, which would preserve tirz's stronger appetite suppression while bringing in reta's glucagon effect. Has anyone here done something like this with those same goals in mind?

There isn't really a 4mg threshold for reta. It starts working right away. Begin at whatever point feels right to you, then titrate in a way that seems sensible and reasonable for reaching your goals.

My wife is currently on 10mg Reta, and we recently introduced 2.5mg Tirz on top of it. That addition appears to have broken through a significant stall and given her a fresh push. Whether she stays on this combination long term is doubtful, though — once she reaches her target weight, she'll likely move to a maintenance dose of Reta, the same as I do.

You could attempt to break the tirz habit....plenty have given it a shot, but you'll come back.... 😎 😎 👍🤣

That's more or less the path I followed — I started on tirz and later brought reta in rather than pushing my tirz dose higher. Each week's experience goes into my pep planner, which lets me work out which compound is responsible for what.

I'm doing fine, no problems on my end. The queen, though, could be a different story. It does appear to be working — she stepped on the scale today rather than Tuesday and was down a pound. I'm not sure why, since I told her every Tuesday, and I bet she'll be down another pound by then. We're also no longer being angels about food and booze.

Getting her to do a weigh-in will make a huge difference.... 👍 🙂
