Split dosing... Maybe I need to titerate up...

Sooooo just want to seek some advice & see what others have experienced.

I Was on 7.5mg Tirz compound however I was getting side effects so decided to split dose 3.75mg.

First few split doses went down well, less side effects and hunger seemed okay... however last two doses I feel haven't done anything for me.

Took a dose last night, felt okay in the morning, come today late afternoon and I feel I could eat a horse. Literally had my meal, then went for some more, and carried on bingeing till I felt guilty lol!

Maybe I need to go up a dose...
 
If you look on glp plotter and input your dose, you can compare it to your full dose once a week. When the plots are overlaid you will see that the twice weekly dose has lower peaks and higher troughs than the full once weekly dose. Hence the easing of side effects. To get the same peak on split dosing you need to increase the dose slightly.

Others try to ride out the spikes in appetite. I find that my appetite is increased along with eating carbs, I’m assuming a glucose spike triggering my appetite. But I don’t wholly exclude carbs otherwise I get tired and fine exercise harder. It might just take a bit of dose and diet adjustment.

I also make sure I have plenty of fruit and raw carrots to eat when hungry.

Sides can be a limiting factor, but if they’re not, titrate away.

How long have you been on 7.5mg?

What are you stats stating, now, and goal?

Area under curve/average level is the same for a once weekly or split dose.

Typically it’s thought that peaks drive sides while troughs have to be high enough for efficacy.

If it is unusual, and not yet time to titrate, it can be caused by just not eating enough.

Also, if you eat a lot of refined sugars or carbs it can do less for satiety than proteins.

You might just need a cheat day or more protein.

That or its time to titrate..

Are you still losing?

Started mid Feb at 262lbs, now 227lbs, recently been struggling to budge weight too... Losing gaining the same LB.

Started 7.5mg on 2nd of May, didn't realise I've been on it that long... Definitely feel like suppression is nowhere near as strong as it used to be... Maybe I will increase doseage and see how I get on, will map it on GLP1 and see...

8 weeks without titrating and not losing- it is likely time..

Yeahhh I defo eat a mix and always have a cheat day on the weekend nothing crazy no bingeing but will eat out or make a dish u enjoy. Realised I started 7.5mg at very start of May sooo maybe it's time...

here is my titration and loss on Tirz- after 6 weeks at 7.5 i upped it to 9

I see, defo going up. Still will continue split dosing but will check the GLP plotter to decide on a dose.

Also curios what's your calorie intake? And how much are you working out?

My main activity was targeting 10k steps a day, many times I would be out at 10pm just to get a final lap in the neighborhood.. I was certainly dedicated to my goal.

As for calorie, I targeted 1200 kcal a day and 120g protein and consistently was within 100-200 of that goal.

Tirz makes it easy to do the things you already know will work, even if they never have before.

Also took supplements.

I figured if I was going to cut my calories to less than 50% of previous that I had better at least think about nutrients, fiber, and protein.

Yeahhh i was targeting 1300 cal a day, it was easy when the tirz was affective. Think I need a reset, write all intake down, note steps, etc and at least that way I can see where I'm going wrong....

This physician says you should not split-dose as it may lead to receptor down-regulation & desensitization. (4 minute mark).

Well, he has his agenda, too.

I for one tried staying at one-weekly doses because of this idea of desensitizing receptors, but it led me nowhere. So it was the lesser of two possible evils to stick with splitting.

At the time I will have desensitized my receptors for the highest official or unofficial dose of tirz I can stand, eloralintide will hopefully be widely available, so I can give rest to my glp1 and gip receptors for quite a while, maybe a year or so. Switching back and force yearly may be a concept for long term use/maintenance. (Just my own blind theory, not proven to work of course!)

Until then I am enjoying the quality of life I have with splitting a proper dose vs. ups and downs between side effects and urges to eat.

Microdosing above all! 🙄 Sounded good in theory, failed praxis in this subject.

Yes, and to be clear, I have no opinion, but thought I'd pass on that data point. I have found his youtube channel to be exceptional.

There is exactly no scientific evidence to support the idea that tolerance to the long term weight loss effects of GLP's exist. Tolerance to gastrointestinal side effects and slow gastric emptying occurs slowly over several months, but average weight is a flat line over 4 years of follow up on a steady dose of tirzepatide, and this would not be true if receptor desensitisation or tolerance was real.

The closest thing to some sort of long term loss of effectiveness is a mouse study where semagltutide was stopped and started repeatedly, and the mice allowed to regain weight while off the drug, and over time weight loss was reduced each time ozempic was restarted. It only suggests that stopping and starting repeatedly might be a bad idea, but it has not been replicated in humans.

He also seems to be advocating very low doses for some odd reason, this is also not supported by the science, it may be reasonable where only small weight losses are needed, but the evidence is crystal clear that weight loss is dose dependent up to the maximum recommended doses. He needs to read the large numbers of scientific papers written on these subjects, having a medical degree or even working in that area does not make you an expert if you do not read the literature.

His youtube video has the study links supporting his claims on receptor down regulation / desensitization from split dosing.
