Right now I'm running Reta at 4mg/wk, and once I get blood work done in a few weeks I'm planning to add Tesa. I realize that a glp makes reaching a fasted state take longer, and that fasted state is needed when you run something like Tesa or CJC-1295. My shift is at night, and my last meal, which is a protein shake, is normally at 3AM. My plan is to pin the Tesa at about 8AM, right before bed. So here's my question: while on a glp, is that amount of time long enough to be in a fasted state? I'd really appreciate any advice or shared experience.

Why not take it as soon as you get up? That's my routine. I used to dose at night right before bed, but I decided I didn't want to take a chance on any of the problems that could come with that.

That is plenty of time, particularly when a shake is all you are having. My own routine is to pin tesa 3 hrs after the final meal of the day, right before sleep.

That's just bro science making the rounds again — fasting isn't a requirement for tesamorelin. Just pick a time that works for you and dose it once daily.

Is HGH really the only growth hormone that doesn't have to be taken while fasted? People often say insulin is the enemy of growth hormone, so doesn't that mean it has to be taken fasted?

My understanding was that fasting is necessary for the GHRH-related effects to occur — or at least that's the message from every talking head online. A quick Google search turned up the following: FDA Label Note: The official FDA-approved labeling for brand-name tesamorelin (Egrifta) centers on once-daily subcutaneous dosing and does not expressly require fasting, although clinical consensus and optimization guides tend to recommend an empty stomach.

Calling it "clinical consensus" mostly dresses up what amounts to peptide vendors and medspas.

Here's something worth knowing: for individuals dealing with metabolic conditions such as obesity or PCOS, taking GHRH analogs while fasted can actually blunt the growth hormone response. Those same people tend to see a markedly stronger response when the GHRH analog is taken after eating. This paradoxical pattern seems to hold even once weight has come down and body weight has normalized. On a weight loss forum, that feels fairly pertinent.

What's interesting is that co-administering Narcan with the GHRH restores a normal response, meaning no GH suppression while fasted.

I wasn't aware of that at all! Looks like I need to look into it further. So it seems I'll be scheduling lab work more often than I originally expected. Since I'm clearly a beginner with GHRHs, thanks for sharing this.

😂 Yeah, honestly.. that checks out once you frame it that way.
