Not medical advice - just what has worked in my case.

My Dosing Schedule

There was no rushing the increases. My body needed time to catch up.

Weeks 1–5: 1 mg once weekly

Weeks 6–9: 2 mg once weekly

Week 10 onward: 4 mg once weekly

I've been sitting at 4 mg for a good few weeks now and intend to hold it there until the goal is reached, then taper down eventually.

My Diet...

No complicated meal plan, and no whole food groups cut out. A handful of simple principles instead:

Make protein the priority at every meal.

Keep the calories sensible without going hungry.

Get plenty of water in.

Stop eating when satisfied, not when stuffed.

A typical day goes roughly like this:

Breakfast!

Premier Protein or a Fairlife shake

Sometimes a homemade PE Science vanilla shake, made up with Fairlife 2% milk

Snack Time... Between Breakfast and Lunch

Either,

2 hard-boiled eggs, or a serving of low fat cottage cheese, or a Protein bar when I'm out and about

Dinner!

Chicken breast, somewhere between 120 and 150 grams (now and then fish), boneless and skinless, baked with no added fats

A big pile of vegetables - peas, carrots, green beans, broccoli or baby corn

Nothing fancy. Just consistent.

Retatrutide has without question cut my appetite down, but good choices are still on me. Some days that's easier than others.

There have been weeks where the scale hardly moved, and a couple where it went the wrong way. Rather than lose heart, I stayed with the plan, because weight loss was never going to be a straight line.

The biggest lesson: consistency beats perfection.

My Results this far,

Across just 15 weeks, I've lost over 75 pounds.

More exciting than the number:

Clothes that used to be tight are now too big, energy is up, everyday tasks are easier, and for once this feels like something I can keep up long term.

My Advise..... If you care lol...

If you're only starting out, don't measure your progress against anyone else's. Build a routine you can actually live with.

Protein, water, stay the course. Some weeks you'll drop a lot, some weeks nothing.

Just keep showing up.

Back when I started, 15 weeks ago, I hoped this might help change my life. I now believe it already has.

Putting on 15lb between week 14 and 15 makes no sense to me.

Dropping 5lb every week for 15 weeks from a 386lb starting point is a risk.

490 lbs was my heaviest. From there I got down to around 400ish, then started reta, went hard at it, and the weight is falling off like crazy!

I assume you know what the word crazy implies for a gallbladder?

15lb in week 14 after putting 2lb back on the week before is wild. That works out at a 5k calorie deficit every day, assuming 40% of what comes off is lean mass.

My intake runs between 800 - 1200 calories a day - plenty of lean protein and vegetables. So far I feel completely fine, and my bloodwork is at last beginning to level out and normalise

Well done on the weight loss. For someone starting out at 490 pounds I have nothing to offer advice-wise. I haven't walked a mile in your shoes. Watching the TV show, my 600 pound life, the people who were determined shifted incredible amounts of weight in a very short time. Your story strikes me as an interesting one. Do keep us posted.

75 pounds inside 15 weeks. That is madness. Respect.

The first goal is a weight that starts with a 2.... As of Sunday I'm 8 pounds from it.... after that comes the 100 pounds lost goal... and eventually, hopefully, right around 200 pounds. It looks a long way off, but so did my first 50 and then 75, so I know it's attainable!! Thank you for the support!!
Genuinely impressive. You are clearly putting the effort in. This is not a case of taking a drug, sitting back in the recliner and waiting for the pounds to fall off. Nicely done.
