Neuroinflammation: An Overlooked Factor in Chronic Pain?

Lately I've been digging into neuroinflammation, particularly its connection to chronic nerve pain, sciatica, and injuries that never quite seem to resolve.

What strikes me is the idea that inflammation involving the nervous system may be able to sustain pain signaling even after the initial injury has healed.

ARA-290 and KPV have caught my attention recently, and they've made me question how significant neuroinflammation really is in long-term recovery.

Peptides for tissue repair get discussed constantly, but calming inflammation around the nerves themselves gets far less attention.

Has anyone else looked into this? I'm curious whether anyone has investigated peptides aimed specifically at neuroinflammation, as opposed to general inflammation or injury recovery.

I'd really like to hear what others have come across, whether that's research or personal experience.

Research in this space runs into a lot of obstacles. A dependable scan capable of revealing brain inflammation simply doesn't exist. The closest options we have are either something that hints at the activation state of Glial cells or a blood based volume measurement that looks for water, yet both are fairly indirect, given that the brain cannot swell. Biopsies of the brain are off the table for clear reasons, so identifying this problem in living people is a struggle, and these processes probably act differently after death — and honestly, apart from leftovers of the problem (tau and such), not much has turned up.

Another major hurdle is getting substances into the brain, since the BBB only permits small proteins through. Without visibility, demonstrating impact and efficacy is hard, and so is targeting whatever we theorise may be going on. Compounds can be tested on cells in a dish, but human trials so far have mostly come up short. It's among the theories floated for ME/CFS and Long Covid, though it has proven one of the tougher ones to demonstrate that there is even a chance it is occurring. Seems like it's still a long way off.

My toolkit includes both ara-290 and KPV. Sciatica has been a lifelong problem for me. Roughly 1 year back I completed a run of ara-290, using 1 10mg kit dosed at 4mg per day. Within something like 2 weeks, my sciatica vanished entirely. It stayed away until roughly 2 months ago, when it came back quite mild and only now and then. As of yesterday I wrapped up an other 10mg kit, and once again it has disappeared completely.

KPV I have taken on its own and as part of KLOW. For assorted arthritis type joint pain it appears to perform about as well as ibprofin. Pushing the KPV dose past 500mcg brings on restless leg syndrome. I run 8 week cycles of KLOW, which I value for a whole bunch of different things.

Are you saying 4.0 mg each day?
