Has Tirz Ghosted Me After a Three-Week Break?

Sorry for the essay, but has anyone else had Tirz suddenly ghost them after taking a break?

Tirz and I had a rather dramatic start. With my first pen, I injected the full 2.5 mg dose and nearly ended up in the ER because my lower esophageal sphincter already doesn’t do its job properly. So I restarted with microdoses and increased by just a few clicks at a time. After six months, the highest dose I had reached was 2.4 mg.

I always had pretty strong side effects: terrible heartburn and reflux, gastritis, occasional abdominal pain, extreme constipation… all the fun stuff. But I kept going because having the food noise finally shut up was absolutely wonderful.

Then, in May, I had to stop for three weeks because of a virus. I gradually restarted and increased the dose again, but now I don’t get any side effects at all.

Great news, you might think… except the positive effects have disappeared too. No control over food noise, no feeling of fullness, and I’m hungry.

Yesterday I finally “bravely” increased to 3 mg, and honestly, I might as well have injected tap water.

Has anyone else experienced this after a break? Do you clearly feel when Tirz is working, or could you take it without noticing much difference?

Before the break, I could always feel both the good and the bad effects very clearly. Now it feels like I’m taking absolutely nothing. Tirz, where did you go?
 
There have been a nunber of studies that show if you stop and come back, you will likely experience exactly what you are, less effect, harder to get it to do anything. You'll likely have to up the dose to get to your new higher level for effectiveness.
 
Are you on grey tirz? If so, is it tested fur purity and strength? Otherwise, ot seems like you’re tolerating it now. It still titrate up slowly, remember you hit dose equilibrium after 4 shots of the same dose. Increase after that, try going by pharma titration 2.5–>5,etc.
 
Um, yeah, it is not meant to be stopped and started and that's basically what happens every time someone thinks it's a good idea to stop their medication, then hop back on.

If you search this board and reddit it's FULL of countless people who have done this and then are completely shocked by their experience.

Fwiw it really sounds like this is not the medication for you, and that's no small part of your problem with proper adherence

At least this gives me some hope that gradually increasing the dose might bring the effects back.

Until yesterday, I had only been increasing by a few units at a time because I was afraid of going higher, given the very strong effects Tirz had always had on me.

Yes, it’s grey Tirz, but I do have test results for both purity and strength. I still wondered whether the vial might be the problem, so I switched to one from a different batch and a different supplier, but so far it feels exactly the same. That’s why I’m starting to think the issue may be my response rather than the product.

You’re right that I haven’t stayed at this new dose for four shots yet. Going directly from 2.5 to 5 mg would honestly terrify me, considering how strongly I reacted to Tirz in the past, but I’ll continue titrating up slowly and give the dose enough time to build up. Thank you! 🫶

I understand your point, but just to clarify, I didn’t stop because I randomly decided it was a good idea or because I wasn’t committed to the treatment. I had a severe virus and was forced to pause for three weeks.

Before that, I had been taking it consistently for about six months and was an extremely strong responder, even very low doses gave me significant appetite suppression as well as very strong side effects.

Many of the examples I’ve found seem to involve people who were not hyper-responders at such low doses, which is why this complete change in my response surprised me enough to ask about it.

It may ultimately turn out not to be the right medication for me, but I don’t think it’s fair to frame the interruption as an adherence problem or a casual decision to stop and start.

With 3 weeks off, you have allowed your peaks to fall and lost some of the half life..

Although cycling is not advised anecdotally many have had to do this for surgery and some have used it as an effective maintenance.

With your health issues I would move forward with caution.

I was comparing how my current 3 mg dose feels with how 2.4 mg felt before the break, without considering that back then I had months of accumulated Tirz in my system. Seeing the numbers makes it click: 3 mg now is not yet equivalent to the exposure I had at steady state before the interruption.

Given how strongly I reacted in the past, I completely agree about moving forward cautiously. I’ll give this dose time to build up instead of rushing to increase it.

At no point in six months did you reach the clinical starting dose, and you seem overwhelmed by your response to the medication. Again, this may not be the appropriate medication for you.

Being terrified is not a healthy place to be while taking a medication.

Tirze builds up in your system, so it may take awhile to catch up to where your level was before. GLP plotter.com can help you see that on a graph.

Did you start a new vial? If not your old one may have gotten weaker ( some say that happens) or if new it may be less concentrated than the last one. They aren't all the same.

I hope it starts working again, and thanks for sharing.
 
Be careful like mentioned above you might need to wait for it to get back into your system. Everyone is different. When I increase I only do by .5mg because I dont want to get bad side effects

Have you tried Reta yet? Maybe that will be the better option for you?

Hope it gets back to working for you & that you're feeling better from the virus you had!! 🙂

I’m fully aware that I never reached the standard clinical starting dose: that was deliberate, because I didn’t need to. Microdosing worked extremely well for me, allowed me to reach my goal, and I’m now only interested in maintaining the result without unnecessarily pushing the dose too high for someone who responds as strongly as I do.

The tone of my post was intentionally exaggerated and lighthearted. When I said I was “terrified,” I wasn’t literally describing an unhealthy mental state around the medication.

Tirz worked exceptionally well for me for six months. My question was specifically about why its effects seemed to disappear after the forced break, because it felt strange to have developed such a dramatic tolerance so quickly. So far, @Skidude explanation about the loss of accumulated levels and the time needed to rebuild them is the most logical one and directly addresses what I was asking.

Thank you so much! This explanation makes a lot of sense. I was comparing my current 3 mg with how 2.4 mg felt before the break, without considering the months of accumulation behind it.

I also suspected the vial, so I tried one from a different batch and supplier, with the same result. I have purity and strength tests, so the accumulation explanation now seems the most likely. I hope it starts working again too! 🫶

Thank you, that’s so kind! 🫶 I think you’re right, I need to give it time to build back up. I also increase in very small steps because I really don’t want those strong side effects again.

I’ve actually bought Reta but haven’t tried it yet, as I’m still researching it carefully. My only hesitation is the possibility of increased appetite or food noise, especially now that I’ve reached my goal and mainly want to maintain it. And thankfully, I’m completely over the virus now!

That’s a good point! Splitting the dose could smooth out the peaks while keeping the same overall build-up. If the side effects return as my levels increase again, that may be a very useful option. Thank you so much! 🫶

Wow! You reached goal Congratulations!! You don't have to worry about it then. Just take it slow & do the lowest dose you can maintain on. No need to experience any crazy sides or anything like that. Enjoy being at your goal & not feeling horrible!! That's Amazing!! 🙂

Thank you so much! 🫶 Reaching my goal is wonderful, but I don’t think I’ll be able to maintain it if Tirz no longer helps with the food noise. I’m very careful with my diet and calorie balance, but episodes of binge eating can undo several days of effort.

That’s why I’m disappointed that the effect hasn’t returned after the break. However, thanks to the helpful insights you’ve all shared and the better understanding I’ve gained from your experiences, I’m confident I’ll find a way forward.

The options I’m considering are increasing my Tirz dose rather than maintaining the current one (which feels a little strange now that I’ve reached my goal, but I’ll gladly do it if necessary) or looking more carefully at whether introducing Reta might help.

I'm almost at 6mo now & just halfway to my goal. Working much slower. I was 5mg Tirz & 3mg Reta, then switched 4mg Tirz & 4.5 Reta. No appetite (but I didn't really have one before starting). Something is just broken, ha ha. Still barely loosing & can barely eat so not sure if I have to increase again or play with doses. Still have about 26lbs left to go. I think the Tirz helps with pain & inflammation (for me) more so I don't want to stop that for sure!! 🙂
