In a lot of ways this looks much like tirzepatide, except that it is a biased agonist towards cAMP at both GLP-1 and GIP, whereas tirz is only biased at GLP-1. Weight loss rates come out pretty similar too, and the low dropout rate is interesting. I could not locate the actual paper, so I have no idea what the side effect rates looked like. Since weight loss had not plateaued at 48 weeks the final results are likely to be better.

Where the newer GLP drugs that are not triple agonists will all end up is very hard to know; reta is simply more effective, and the triple agonists still in development will no doubt be too. Going on the low dropout rate alone this one might have lower side effects, which would give it something to set it apart from tirz in the market. As for the injectable GLP-1 only drugs in development, I cannot see any realistic route to market success for them unless they come in cheap and land quickly, before dozens of them exist.

I know next to nothing about how peps get made, but what I've read says eloralintide was a bit of a pain to make, for reasons my tiny brain didn't catch. Maybe this one is easier? Fewer aminos in the chain, or whatever.

I know next to nothing about how peps get made, but what I've read says eloralintide was a bit of a pain to make, for reasons my tiny brain didn't catch. Maybe this one is easier? Fewer aminos in the chain, or whatever.

What makes it awkward is a more complicated chemical end group, one that can take different forms depending on how it was synthesised. Take thalidomide back in the 60's / 70's: one stereoisomer was fine, the other caused birth defects. That is an extreme case, but a stereoisomer is essentially a mirror image, and biologically it makes a big difference even though the masses are identical - HPLC would not have told them apart without some sophisticated PhD level chemistry. Synthesising the peptide chain is the 'easy' bit.

With several routes now available for improving half-life, my bet is that what comes first will be peptides equivalent to the ones we already have, but modified so they dodge the existing patents.

Versions aimed at mutations in "low-responders" may turn up, though they will be difficult to develop. Resistance from patients who want "the real drug" is also likely, as cardiology has shown.
