I've spent many weeks at 250mcg, then a shorter stretch at 350mcg, and now I've stepped up to 500mcg. Right now I'm sitting right at the edge of tolerable. I like the way it's working, but any increase at all would push things into unpleasant territory.

So here's what I'm wondering: should I repeat the same dose for another week, trusting that my body will begin adapting and everything will stay manageable? Or should I drop it back down, since buildup could carry me past 500mcg into risky territory? My goal is simply to hold things exactly where they are.

I realize there's no clear answer, but I'd like to hear what others think.

My first Cag shot was last week. What’s driving the increase for you — did the lower amount stop working, or was it never doing much to begin with?

Last week I injected 250mcg, and 350mcg went in last night. So far I’ve gotten nothing from it. The idea was to stack it alongside a low maintenance amount of Reta.

At 250 I didn't notice anything, at 350 there was a slight effect, though I thought it could be stronger. On top of that, I take 12.5mg of Tirzepatide.

Right now I'm pretty much in the same boat. I injected 266 mcg of cagri this week, and reta has been part of my routine for 8 weeks at an unchanged low maintenance dose of 1 mg. So cag is only acting as a complement for me. What I've observed is that my appetite and hunger are at zero, and they've stayed at zero from the moment I began reta. Once I added cagri, though, I can't say for certain yet since it's only been a few days, but my digestion seems to have slowed down even more compared with when I was only taking reta. It feels like food lingers in my stomach longer, and for me that's a pleasant sensation. I enjoy it because it keeps me full. Any time I begin eating something, I become full right away, more so than when reta was the only thing I was on. It's kind of wild at this point.

My advice is to remain at 500 mcg for a number of weeks, since cag needs some weeks to stabilize inside your body before it hits steady levels.

I began with 500mcg, which in hindsight may not have been the wisest choice. Still, since I'm already at that level, I'm continuing forward. My 3rd 500mcg injection was on Monday.

My sense is that going from 350mcg up to 500mcg doesn't represent a huge leap. Because Cagri's 1/2 life is longer than tirz and reta, you're right that the full impact of that dose wouldn't have kicked in yet after just your first 500mcg shot. That said, it's doubtful this would land you in a "danger zone." Honestly, I don't even know what that term is supposed to mean.

Looking at the phase 3 cagrisema trial, the usual titration schedule was 250mcg for 4 weeks, then 500mcg for 4 weeks, followed by 1mg for 4 weeks, and so on. Every step there is a 100% jump. Your own increases have been far smaller by comparison, so I expect you'll be fine.

Fair enough — calling it a danger zone was overstating things. What I was really getting at is that any stronger effect from the Cagri would leave me unable to eat anything at all. It's a tempting shortcut for faster weight loss, but obviously that isn't the ideal outcome.

Alright, that makes sense to me now. And yeah, I'm with you — cagri can definitely do that. It's tempting to simply skip eating, but like you said, that isn't really the right approach. Hitting my daily calorie target has been tough, and it was already set at a pretty big deficit. The only thing I can really do is prioritize protein.

What led you to pick that amount? Drawing 266 consistently sounds like it would be tricky, doesn't it?

Haha, right — 266 micrograms is definitely an unusual number. I was aiming for 250 mcg, but here's how it shook out: my cagri comes in a 10 mg vial. After mixing in 3 ml of bac water, hitting 250 mcg would mean pulling the syringe to 7.5 units, which is a real pain to measure precisely. So instead I just go to 8 units, and that lands me at 266 micrograms.

I already know how standard titration schedules work, so that's not what I was after. What I wanted to know is whether repeating the same amount for a second dose tends to feel stronger than the first one did. My worry was that it would be too much, to the point where eating would become impossible. Either way, I injected it last night, so I guess we'll see what happens!

I've been on this for more than a year, with breaks here and there. My starting point was low—0.1mg—and I stayed there for a few weeks just to make sure nothing bad happened. After that, I moved up gradually, roughly 0.1mg every 3-4 weeks. These days I'm at 1mg weekly. When tirz or reta stop delivering their usual punch, cagri fills the gap nicely. There were stretches where I had to pause cagri because the nausea was relentless. All of these peps need a few weeks before their real effect shows up, so don't rush your increases. And once you're at the max dose, there's nowhere left to go: your body has adapted and you'll notice the stall.

Answering my own question here: nope, when I repeated the same dose in week 2, the sides weren't any harsher than week 1. Actually, my appetite is stronger than it's been in a long while — even before I ever touched Cagri. No clue why that is, but we keep moving forward.

So you mean the dose has stayed the same for more than a year? For me, reta has been at the same dose for 9 straight weeks — that's how long I've been on it — and it's 1 mg each week. Then just last week I added cagri at 266 mcg. And here's the thing: at this same reta dose, I'm now feeling more effect than I did at the beginning. My guess is cagri will behave the same way for me. I'm taking cagri alongside reta, and I predict the magic won't fade. I also doubt the body builds tolerance to it. I haven't come across that in the scientific literature, and there don't seem to be studies on it. That said, it clearly differs from person to person.

I don’t follow a fixed schedule with it the way I do with Tirz and Reta — it’s strictly an as-needed thing for me. Up to this point, .25 has been all I’ve required, and it tends to carry me a bit past a week. Fridays are when I take my Tz/reta, and I’ll only reach for the Cagri when my appetite isn’t being suppressed enough. Since appetite suppression is the sole purpose Cagri serves for me, it only comes out when the Tirz isn’t doing the job. Right now I’m 10 days out from my last Cagri

Each month I've been bumping Cagri up roughly 0.1mg. For the past 3 months I've stayed at 1mg. Reta is at 10mg. Over 2 years I've dropped 102#, with 5# left before I reach my goal.
