MCAS, ME/CFS and conditions in that family appear to be responding well to GLPs. Patients have described substantial symptom improvement — less fatigue, clearer thinking, lower mast cell activation — once these medications were started, even at the low "microdose" end.

A COVID infection left me with circulation problems and neuropathy, both of which have eased. Layering up as carefully as I can, I still could not tolerate time in the cold without my fingers and toes burning or going numb. Personally, 1 wk of tirz at 250mcg has been life changing.

Doctors working in ME/CFS/FM and long-COVID are conservative by nature — conservative not in what they prescribe but in how they bring it in. They phase treatments in gradually so as not to spark a bad reaction. With at least these two doctors, that trial period looks finished, and they are putting them in early in their protocols.

Dr. Dempsey said, “It was pretty mind-boggling to see the response and the fact that it was global, it was systemic, and pretty quick”. She, like Dr. Kaufman, has moved GLP-1 agonists way up on her protocol. Both are considering using it very early with their patients. Why, Dr. Kaufman said, make the patient wait, particularly since at very low doses, side-effects are usually not a problem?
