If you do have kidney issues, there are treatments you can take to slow the decline like SGLT2 inhibitors. The problem with the kidneys is that they don’t hurt when their function declines. I’d be damned if I would waste my time going to a specialist when for $40 I could test:

You’ve figured this out already but the normal eGFR on the CMP is useless if you’re running creatine and working out a lot. Both are serious confounders.

Go get Cystatin C drawn and calculate your eGFR off that. Not the combined creatinine/cystatin c eGFR, just the cystatin c one. That’ll be much more accurate.

On a recent draw I had a creatinine level of 1.3 and a cystatin c of 0.68. That corresponds to a creatinine estimated eGFR of 73 (not great) and a cystatin c estimated eGFR of 124 (that’s reta baby). Only the second one is close to accurate.

I DO supplement with 10g of creatine a day, in a divided 5g dose. I do take KLOW and RETA. Just throwing that out there. I've been taking the pep's for about 6 weeks, but I've been taking the creatine for years. My creatine on previous labs was also in the normal range. I'm not sure if that helps you at all, but thought I'd share my own personal experience.

Same here. I've been taking 5-10g of creatine a day for almost 13 years and just had my annual blood work done last month. Results where the same as ever for creatine levels - totally in the normal range.

It might be just how our bodies metabolize or process the creatine differently, but there's obviously enough folks that take it daily for years without any issues.

It's unfortunate some folks do have issues with taking it, but it would be presumptuous to claim it's bunk as a supplement because of that.

Do you walk 5 miles a day and lift weights 5 times a week? And lifting the max weight your body can stand? Look at what creatine looks like with active high to moderate exercise. It’s fantastic. I’ve been doing creatine and high exercise since 2016. Agree fully. Everyone reacts differently like you said. I have zero worries about my heath.

Agree 100%! I belong to lots of FB groups for muscle building, etc. and there are lot of people who say creatine is a waste of money, that it gives them nothing but the runs, and/or they feel no difference. As you say, perhaps for them it is a waste. For me, I don't notice any bloat or other side effects, it lets me push harder in the gym (I've tested this many times), and at the 10g dose, I stay sharper/have less brain fog. Unless for some medical reason I would be forced to stop it, I'll take creatine for the rest of my life.

Spot on! I recently had very similar numbers. I take creatine and eat a high protein diet. My creatinine level averages around 116 but has occasionally spiked to above 1.3. The creatinine estimated eGFR comes back upper 60s/low to mid 70s. I added in the cystatin c estimated eGFR test and it came back 112 (cystatin c was .70). Really big difference! BTW, I had to have the test added. The primary care doctor wasn't aware of it.
 
Prior to a blood test, I stop taking creatine supplements for one week to avoid any potential impact on the results. My results always have been in normal range for kidney function.

I also find that lab timing affects that creatinine quite a bit. I almost exclusively have draws done first thing in the morning (I already have my next three panels scheduled so that I get that first slot). But my eGFR will look a lot better if I do a draw at any other time of the day when I’m not as dehydrated as I am first thing in the morning.
