Based on what people here have gone through, how frequently do imported vials turn out to have unsafe endotoxin levels?
 
Up until roughly 2 weeks back, the leadership at a few large labs claimed testing for this was pointless, since they had yet to encounter any significant failures. That was before Jeep, so at minimum it has been uncommon. The open question is whether Jeep was an isolated incident or whether this has turned into an ongoing issue.

But it turns out Jeep wasn't the only one. Those light orange caps on T30 ended up in multiple vendors' stock — or at least, that's how it was. On the bright side, skipping endotoxin testing is a thing of the past, and far more endo tests are happening now. A number of vendors now treat elevated endos as grounds for a refund, so things are moving in the right direction.

It's happening with greater frequency lately. Testing is a wise precaution.

For those of us who are new to this, that raises something worth asking. How quickly does word actually travel through the whisper network? Is there any sign that the big labs were paying attention to the Jeep case, or that other producers even care?
