New to This: PCOS/PMOS with Insulin Resistance — Looking for Guidance

Hi everyone! Around 3 years back, I received diagnoses of Hyperinsulinemia (insulin resistance) along with PCOS/PMOS, and I've now made the decision to pursue this path for managing it. Zepbound was what I used for 2 months, and I really liked it, but then my insurance cut off coverage, and trying to figure all of this out left me feeling overwhelmed. I switched to wegovy, and I've disliked it intensely; I experienced muscle loss, significant hair shedding, and stubborn acne on my shoulders/back that won't go away. For 3 years I've been on metformin at 1500mg, and it seems to help a little with insulin sensitivity. Spironolactone has been something I've cycled on and off at every dose for at least 6 years, and I'm ready to try something different. My goal is to drop no more than 30lbs, and what I really want is radiant, healthy skin and hair. I've been looking into Tirz or Reta for weight management, plus GHK-Cu & KPV for skin and hair health. Learning about this kind of thing on my own is hard for me, but I'm putting in my absolute best effort. Any and all suggestions or input to help me dive fully into this journey would be so appreciated. <3
 
Hello! I'm not sure whether this is something you've possibly come across before or perhaps already used, but do you know about pioglitazone or the other medications in the TZD class? In case these are new to you: this drug class consists of highly potent insulin sensitizers. They redirect adipose storage toward sWAT rather than VAT, favoring the lower body and particularly gluteofemoral sWAT. On top of that, they enhance the general metabolic performance of sWAT.

I've come across reports of individuals stacking low-dose retatrutide with pioglitazone for body composition purposes, and I'll likely give that a try myself before long.

has mots-c/aicar ever crossed your mind? these are AMPK activators, similar to metformin, except they don't cut ATP production in order to work. they're believed to assist with insulin resistance.

This is all new to me, though it seems worth investigating. The majority of my weight sits around my middle. If I don't deal with this soon, I worry my insulin resistance could progress to NAFLD.

I haven't given them any thought. I know some people take mots-c, though that's as far as my looking into it went. I should probably dig deeper into that.
